  1. NAF Data Collection Template (2022-2023) The Quality Center, located in the Academy Support Hub (ASH), is the home for NAF s annual data collection process. Academy data are collected each year to populate NAF s Academy Assessment (AA), generate academy reports, and ensure academy and user information is accurate. Instructions: Use NAF s Data Collection Template as a guide for collecting the required academy data. Once the required data has been compiled for each of the forms listed below, log into the Quality Center in ASH and use the template as a reference to enter the data online. The 2022-2023 deadline for NAF s annual data collection process is December 2, 2022, for academies completing the AA and October 21, 2022 for Roll Forward academies. Form Description 1. Work-Based Learning List of Work-Based Learning activities from previous SY 9. Social Media Social media used to promote NAF and academy events 10. Roll Forward Academy s Roll Forward status for current SY 11. Finalize Finalize after updating and confirming all required forms above

  2. Form 1: Academy Information Verify that all academy information is current for the 2022-23 school year (SY).   3. Form 2: Academy Enrollment Data fields for the Academy Enrollment form automatically populate from 2022-23 SY NAFTrack entries. Verify accurate academy assignment for students on the List of Students tab. Verify accurate academy student totals on the Student Totals tab. The Automatic option automatically counts a student as enrolled in more than one academy if marked as such in NAFTrack.

Form 3: High School Enrollment Enter the student counts for each grade level and school-wide percentages for race/ethnicity, gender, free/reduced lunch, and English Language Learner (ELL) demographic fields for the 2022-23 SY. Note: For high schools with multiple NAF academies, the first academy to enter the high school s enrollment data automatically populates the other academies data for this form.   5. Form 4: Graduation Profile Enter postsecondary plans for the 2021-22 SY academy seniors. Note: A list of the seniors automatically populates this page from student data in NAFTrack. Select the name of the postsecondary institution the student will attend. Choose Other for institutions that are not listed or contact support@naf.org to add an institution.   6. Form 5: Advisory Board List each Advisory Board (AB) that supports the academy and update member profiles and contact information. Create a new advisory board or Edit/View an advisory board list from previous years. Verify AB members who are active for the 2022-23 SY and update board member data on the Advisory Board Members tab. Verify academies served by each AB for the 2022-23 SY on the Academies Served by this Board tab.   7. Form 6: Teachers Enter the names and contact information for the 2022-23 SY academy teachers. Identified teachers gain access to NAFTrack. Recommendation: Use teachers official (school-assigned) email addresses. Teachers who were active last year are displayed by default and can be reactivated by checking the box next to their name. New teachers can be added using the button.   9. Form 8: Work-Based Learning Verify the academy s Work-Based Learning (WBL) activities from the previous school year and upload the academy s completed 2021-22 SY WBL calendar as evidence. The academy s work-based learning (WBL) experiences include a continuum of career awareness, exploration, and preparation activities that build in sophistication, duration, and intensity.   11. Form 10: Roll Forward Verify the academy s Roll Forward status for the 2022-23 SY. All Certified, Model and Model-Distinguished academies are eligible to Roll Forward and maintain their previous quality level and are exempt from taking the Academy Assessment (AA) if they meet the following criteria: Academy is in year-2 or year-3 of a 3-year data cycle, which begins when an academy takes the AA. Academy updates annually required academy and school-level data by the established Roll Forward deadline. The deadline for this school year is October 21, 2022. Model and Model-Distinguished academies must meet established Thresholds by the Roll Forward deadline to maintain their previous quality level. Academies that do not meet required Thresholds may opt to Roll Forward at a lower academy quality level. Academies that do not meet the Roll Forward deadline must complete the AA. After a 3-year data cycle ends, all academies, including Model and Model-Distinguished academies, must take the AA to begin a new 3-year data cycle.

  12. Form 11: Finalize Clicking the Finalize button on the Finalize form indicates that data entries on forms 1-10 are complete and accurate for your academy for the current SY. NAF s annual data collection must be finalized by December 2, 2022, for academies completing the AA and October 21, 2022 for Roll Forward academies. Form Description Action 1.
