New Plans & Changes effective 1/1/2025 Heritage & Prime networks Plan HSA 1600 is changing to HSA 1650 due to IRS minimum deductible requirements *NEW* Plan 6000 In-Network benefits include: $6,000 deductible (2 x family) 20% coinsurance $8,000 out-of-pocket maximum (2 x family) $40 copay *NEW* Plan 7500 In-Network benefits include: $7,500 deductible (2 x family) 20% coinsurance $9,000 out-of-pocket maximum (2 x family) $40 copay
Plan Choice Matrix Updates Plan 3000 & Choice 3000 removed from Package A HSA 1650 (previously HSA 1600) removed from Package B Added Package C includes the new Plan 6000 & the plans it can be paired with Added Package D includes Plan 7500 as a single option
