Student Health Care Information Student Health Care Information Student Health Care Information Immunizations Incoming 6th graders must have updated immunizations before the start of the school year. Any new immunizations require a Certificate of Immunization Status form from the State of Washington. Life Threatening Health Care Plans (HCP) If your student has a life-threatening allergy, you must have a current 2021-22 HCP with necessary doctor s orders and medication. The date and time to drop off medications to the school nurse will be communicated via ENews. This allows ample time to process this important information for compliance with Washington State law and Issaquah School District regulations.
