Range of Issues Around Health and Housing Lack of awareness, access and availability of community services: More than 800,000 people are on state Medicaid waiting lists, causing unnecessary institutionalization. People aging in place lose housing due to lack of support. Approximately 50% of HUD properties for older adults lack a service coordinator. Affordable and accessible housing stock: 48% of extremely low-income renter households are seniors (30%) or people with disabilities (18%) For every 100 extremely low-income renter households, there are only 40 affordable and available homes Fewer than 5% of housing units are livable for individuals with moderate mobility difficulties, while Less than 1% are accessible to people who use a wheelchair full time. Each year, nearly 900,000 individuals fall into homelessness One quarter of the total homeless population is reported to have a disability* One in three people who are chronically homeless have a mental health disability** 5

Building Connective Tissue to Support Housing & Health Partnerships The housing and health sectors are natural partners to tackle a range of issues, including ending homelessness, helping older adults age in place, and improving health in under-resourced communities. Building connective tissue for effective housing-health initiatives (brookings.edu)

Whats Possible When we work together to align and leverage our work to coordinate affordable, accessible housing with increased access to health and home and community-based services, we create the infrastructure to fully meet the needs of the people we serve. Partnerships don t just happen, however. They need connective tissue an infrastructure supporting frequent and systemic level collaborations to help form the partnership and hold it together over time. Maryland Partnership and Leveraging Opportunities Maryland Health Department, Money Follows the Person Demonstration Project (MDH-MFP) is a partner in the Maryland Partnership for Affordable Housing (MPAH). MDH-MFP, along with the Maryland Department of Disabilities and the Maryland Department of Housing and Community Development: 4 Affordable Housing Programs HUD 811 Project Rental Assistance (HUD 811 PRA), The Harry and Jeanette Weinberg Foundation's Affordable Rental Housing Opportunities Initiative for Persons with Disabilities (Weinberg), the Money Follows the Person Bridge Subsidy (MFP Bridge Subsidy), and Community Choice Home (CCH). CCH is specific to Montgomery County. NLE Grant Activities Integrate a statewide system to support access, sustainability, and participation in Evidence-Based Behavior Change Programs Implement standard screening for SDOH needs across CBOs and Providers Establish a multi-cultural aging network and create a Reimbursement Learning Collaborative for AAAs and CBOs Work with MD hospitals, the MD Primary Care Program and CRISP to document ROI and improved quality of life for older adults and people with disabilities Leverage existing contracts to pilot reimbursement models that will lead to sustainable funding streams. 20
