Social determinants of COVID risk COVID-specific COVID Risk Factors Reduce disparities in COVID cases Reduce disparities in COVID deaths Reduce COVID mortality rates for people of color Reduce disparities and/or prevalence of COVID risk factors, including: Diabetes Hypertension Obesity COPD Improve testing rates Improve PCP utilization rates Reduce disparities and/or absolute prevalence of SDOH leading to worse health outcomes for communities of color: Poverty rates Uninsured rate Homelessness and housing insecurity Joblessness or low- income work Incarceration How far upstream do we want to focus?
