LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT The purpose of the LPHA COVID-19 and Adult Vaccination Supplemental contract is designed to support local efforts to plan, develop and maintain a public health workforce to: Increase COVID-19 and adult vaccination capacity across jurisdictions, including among high-risk and underserved populations; Ensure high-quality and safe administration of vaccines; Ensure equitable distribution and administration of vaccines; Increase vaccine confidence through education, outreach and partnerships; Develop and implement community engagement strategies to promote vaccination efforts; and Utilize immunization information systems to support efficient vaccination efforts
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Funding for a three (3) year contract period February 1, 2021 April 30, 2024 Funding formula based on jurisdictional adult population
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Definitions High-risk Age, gender, race/ethnicity, poverty, overcrowding, pregnancy, certain occupations, certain medications and some medical conditions Social vulnerability Socioeconomic status, household composition & disability, minority status & language, housing type & transportation Underserved Disadvantaged because of ability to pay, access care or other disparities for reasons of race, religion, language or social status
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverable 4.1 Increase COVID-19 vaccination capacity across jurisdiction, including among high-risk and underserved populations: Expand operations outside of normal operating times to increase throughput
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverable 4.2 Ensure high-quality and safe administration of COVID-19 vaccines: Designate an Adult Immunization Coordinator by August 1, 2021 Notify Bureau of Immunization of Coordinator contact Ensure all personnel receive training on vaccine storage and handling, including monitoring vaccine temperatures, vaccine transport and any vaccine specific considerations and vaccine administration. For each offsite COVID-19 vaccination clinic utilize the CDC Patient Safety Checklist 508 and maintain the documentation.
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.3 Ensure equitable distribution and administration of COVID-19 vaccines: LOGIC MODEL - submit an outreach immunization logic model for high-risk and underserved. Include collaborating partners, identify the population being assessed and designate how 10% of the funding will be used for the population at risk. Submit by September 1, 2021. Update annually. Choose one clinic method option to implement: A. Implement adult vaccination clinics to conduct non-traditional adult immunizations clinics for high-risk and underserved communities at least once a month. B. Identify and implement a monthly temporary mass vaccination event in a non- traditional clinic. MUST utilize at least 10% of contract funding to conduct immunization outreach activities and services to high-risk and underserved population
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.4 Increase vaccine confidence through education, outreach and partnerships: Partner, plan and implement vaccination activities with critical organizations, in priority order: a. Long-term care facilities (LTCFs), including independent living facilities, assisted living centers, and nursing homes b. Non-traditional providers and locations that serve high-risk populations c. Other partners that serve underserved populations d. Occupational health settings for large employers e. Churches or religious institutions f. Federally Qualified Health Centers (FQHCs), including Community Health Centers (CHCs) g. Organizations and businesses that employ critical workforce h. Colleges and Universities
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.4 (continued) Increase vaccine confidence through education, outreach and partnerships: Promote COVID-19 vaccine and other adult vaccinations to increase vaccine confidence in racial/ethnic minority groups and increase accessibility for those with disabilities. Enhance and amplify consistent messaging (including translation) using reliable sources including the Missouri COVID-19 publicly available media (print/digital) to promote COVID-19 vaccination among underserved populations. Address vaccine education efforts to include vaccine misinformation, vaccine confidence and vaccine uptake in racial/ethnic minority groups and increase accessibility for those with disabilities.
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.5 Develop and implement community engagement strategies to promote COVID-19 and Adult Vaccination efforts: Implement the CDC COVID-19 Vaccine Confidence Rapid Community Assessment Guide OR a current Community Health Needs Assessment (less than 3 years old) to diagnose potential barriers and identify solutions to low vaccine uptake or vaccine confidence in communities of high social vulnerability. Submit by September 1, 2021 Plan to implement your RCA early enough to utilize the data for your logic model due September 1, 2021
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.5 (continued) Develop and implement community engagement strategies to promote COVID-19 and Adult Vaccination efforts: Engage in at least one quarterly bi-directional conversation with diverse communities to promote COVID-19 and adult vaccinations. Choose at least one option to implement: a. Conduct a county level digital campaign using CDC developed materials b. Provide printed adult vaccination materials to patients c. Implement direct adult immunization mailers (postage paid) d. Develop and print LPHA specific adult immunization flyers for county healthcare providers
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.6 Utilize the immunization information system to support efficient COVID-19 vaccination and other adult vaccinations: Implement at least one of the following options a. Develop and implement an adult immunization reminder recall system b. Develop and implement an Electronic Medical Record c. Develop and implement an HL7 interface with an EMR system and ShowMeVax ImmunizationHL7onboarding@health.mo.gov or 800-219-3224
LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Deliverables 4.6 (continued) Utilize the immunization information system to support efficient COVID-19 vaccination and other adult vaccinations: All doses of COVID-19 vaccines administered must be entered into ShowMeVax either by manual entry or an HL7 interface. Develop a process to input all written vaccination records into the state immunization information system including historical and administered doses.

LPHA COVID-19 and ADULT VACCINATION SUPPLEMENTAL CONTRACT Contract Budget & Funding Provisions Funding for the three year contract is provided through a CDC grant All contract funds must be fully expended no later than May 1, 2024 Underutilization of contract funds may be reallocated with 30 days written notice May reallocate up to 10% of contract budget between budget categories for approved expenditures without a formal amendment Indirect cost not to exceed 10% of the modified total direct costs (MTDC) MUST utilize at least 10% of contract funding to conduct immunization outreach activities and services to high-risk and underserved population
