Why do annual health checks High rates of morbidity and mortality amongst problematic drug and alcohol users Long term conditions copd, cancers, vascular disease, hepatitis C, cirrhosis, amputations, mental health etc Poor uptake of screening programmes Picking up these conditions and acting will make a difference

  3. Case 1 what needs done and how to organise?? Mr FA 42 years old PHx using drugs and been in and out of treatment for past 18 years Acute pancreatitis 2015 Rx Methadone 100 mls/d supervised consumption Attends RESET worker in the practice , but often attends late, occasionally misses appointment but more often late. Gets script from GP after RESET appointment, often very rushed appointment as he late and wants to get to the chemist

  4. Case 2 - what needs done and how to organise?? Miss IR 58 yr Crack and heroin user on and off since 1980 Phx copd, hep c Rx methadone 70 mls/d sdc, salbutamol, lansoprazole Attends RESET and mile end, prescription done by GP in practice attends but often late

Mr WC 61 yrs recent hospital discharge mostly housebound, in a wheelchair, sister brings him to appointments when can, but often unable to do so Attends RESET keyworker in practice or else via phone Rx methadone 23 mls per day , insulin, Lisinopril, Dns dress legs new ulcers Date 21-Aug-2018 17-Aug-2018 23-Feb-2017 31-Jan-2017 16-Sep-2016 16-Sep-2016 Problem Drug addiction therapy C/O: itching Acute osteomyelitis Chronic kidney disease stage 3 Portal hypertension Oesophageal varices in cirrhosis of the liver Associated Text Date Ended mild hep C cirrhosis. grade 1 varix, not bleeding to foot following amputation and debridement 01-Sep-2016 Other graft of skin 27-Oct-2014 22-Oct-2014 25-Sep-2006 04-May-2004 13-Sep-2002 01-Mar-2002 12-Aug-1996 1984 Drug addictn therap-methadone Chronic obstructive pulmonary disease Essential hypertension Arthralgia of multiple joints Type 2 diabetes mellitus [X]Moderate depressive episode Drug dependence Admit psychiatric emergency pos cannabis related psychotic episode pleural effusion 1983 Pulmonary tuberculosis
