Standard Operating Procedure for the Cadet Command Form 104-R, Planned Academic Program Worksheet
Purpose of the CC Form 104-R Organize your academic plan Way for cadre and staff to monitor your degree progress, determine eligibility and ensure alignment with military courses Tool to ensure you complete all required degree and Army ROTC courses to graduate and commission on time.
104-R Process (each semester) New students may want to meet with the ROTC Instructor with a draft prior to advisor Cadet meets w/ROTC instructor Reviewed/Signed Cadet Creates Draft using course catalog Cadet schedules and meets with academic advisor Reviewed/Signed by Advisor PMS Signature Academic Advisor ensures all classes needed for degree reflect on 104-R ROTC Instructor ensures all ROTC classes (27 credits) reflect on 104-R Cadets are encouraged to find an upperclassmen with similar major/school Cadets keep electronic version for future updates

Cadet Command Form 104-R Latest version can be found on ROTC forms/pubs site: https://armyrotc.army.mil/forms-publications/ Form is also on GEB enrollment forms site https://www.marquette.edu/army-rotc/enrollment-documents.php Form can be downloaded and saved on personal computer. It is an Adobe Acrobat form (do not convert it). If you do not have Adobe Acrobat on your computer, you can download a free version FORM MUST BE TYPED!!!
104-R Blocks 1-8 Block 1 Block 1: Fill in your name (Last, First, Middle Initial) For Cadet ID, you can get this from your instructor or the ROO once you are in the system. Block 2 Block 2: Fill in your academic major (ie: Criminal Justice, Civil Engineering, etc.) 2a 2a: This is only necessary for HQ STEM Scholarships. Block 3 Block 3: Fill in the date you filled out the form Block 4 Block 4: Select type of degree pursing (Bachelors, Masters, etc) 4a 4a: Marquette University is the host university, all others mark Host Block 5: Block 5: Calculates how many semesters you need to graduate. Most degrees are Semester 4 yr but STEM degrees can be Semester 5 yr . 5a 5a Input credits for degree (most common is 120, but you will need to verify) Input ROTC credits that do not count for degree (will need advisor input). ROTC is 27 credits Some schools/majors will accept ROTC courses for credit while others do not. The next line will populate with total credits needed for completion of ROTC/degree 5b 5b Input credits completed from school 5c 5c Input transfer credits the school accepted towards degree. This can only be credits accepted your school. You will also need to provide transcripts for these courses for your file. You can annotate these transfer credits in blocks 9 or 11. Put in the course info and mark (T) to indicate a transfer class. 5d 5d This line will calculate credits remaining for degree 5e 5e This line calculates how many semesters you will need to complete your degree Block 6: Block 6: Enter the term and cumulative GPA for each semester completed. . Block 7: Block 7: 7a 7a Select 3 BDE 7b 7b Select Marquette University 7c 7c Select the school you are attending Block 8. Block 8. If your academic school is Marquette, select Host . All other schools select cross town . total. by
104-R Blocks 9 & 11 Block 9 and 11: Block 9 and 11: Each block represents a semester. Select from the drop down menu Fall, Spring, etc then the year. If the drop down menu doesn t have the correct year, leave it blank and pen and ink the year. If you run out of room and can t list all of your classes, continue into block 11 on the second page. Make sure to list the term/year. Term: Term:Fall, Summer, Spring Year: Year:2024, 2025, etc No: No: Course Number in catalog Course Title Course Title: Name of course Hrs Hrs: :Number of credit hours for the class DL?: DL?:Check this is the class is online. You must take at least 50% of your classes in person. See ROO for details. Grd Grd: :grade received for class Total Term Hours Total Term Hours: will self-populate
104-R Blocks 10-16 Block 10 Block 10. You will initial these black spaces each time you are counseled by your ROTC instructor. The terms refer to your semester in ROTC, not that you have been counseled on each individual term. A college sophomore who is in their third semester would initial next to term 1, term 2, and term 3 after the counseling. Block 12 Block 12: Academic Advisor checks based on if your plan is viable. Enter the degree you are receiving with date you will receive it. Block 13. Block 13. Cadet signs and dates Block 14 Block 14. Block 15 Block 15: Academic Advisor signs and dates Block 16 Block 16.
Page 3 of 104-R Read, fill in name, name of academic school, type of degree (bachelors, masters, etc) and sign/date. Your ROTC instructor will review, initial, and turn into the PMS for final review and signature. This form will go into your 201-R file. .
Things to Remember Save it electronically. Each semester you can make minor changes to update. Some classes are only offered once a year. Do not miss prerequisite course(s). If you change a course, ensure the new course meets the requirements that the original one satisfied or meets a requirement that you planned to satisfy in the future. Contracted (or pending contracted) Cadets cannot change major unless approved by Professor of Military Science. You must maintain full-time status while participating in ROTC (at least 12 credits for undergrad) Ensure you include Military Science courses to include the military history course. Use your upperclassmen as a resource
