Health Care for Homeless Veterans (HCHV) The Healthcare for Homeless Veterans Program (HCHV) offers services designed to assist homeless veterans to become active and productive citizens. We meet veterans where they are, provide hope and empower change to help veterans realize their potential. HCHV Mission Statement
HCHV Eligibility Veteran is defined as a person who served in the active military, naval, or air service, and who was discharged or released under conditions other than dishonorable. When someone who is homeless identifies as a Veteran or having served in the Military the HCHV team can aid in determining eligibility Align with HUD definition of homelessness and chronic homelessness

HCHV Outreach Social Workers Provide assessments for all veterans facing homelessness or housing insecurity Social worker and Veteran discuss Housing options, goals, and barriers. Outreach social workers liaison with local community agencies Homeless Veteran call center referrals (1-877-424-3838) Provide outreach to Veterans who are homeless in the community
Transitional Living Programs Contract Residential Services (CRS) and Grant and Per Diem (GPD) Temporary short term housing for Veterans who are homeless Services are provided by community partners through a grant or contract with the VA Case management services focus on assisting the Veteran in transitioning to permanent housing Some programs focus on clinical treatment concurrently with housing stability
Justice Outreach/Re-Entry (VJP) The VJP Initiative: The purpose of the VJP initiative is to avoid unnecessary criminalization of mental illness and extended incarceration among Veterans by ensuring that eligible Veterans in contact with the criminal justice system have access to Veterans Health Administration(VHA) mental health and substance abuse services.
Homeless Patient Aligned Care Team (H-PACT) The H-PACT program provides a coordinated medical home specifically tailored to the needs of Veterans who are homeless. Veterans assigned to H- PACT receive care from a dedicated team that includes a primary care provider, nurse, social worker, pharmacist and an occupational therapist.
HCHV Case Management Provides longer term case management services for Veterans who are homeless Case Management aims to help Veterans obtain permanent, sustainable housing
HUD- VASH Housing and Urban Development and Veterans Affairs Supported Housing (HUD-VASH) Partnership between the Department of Veterans Affairs and Department of Housing and Urban Development Provides permanent supportive housing for Veterans who are homeless through a subsidized housing voucher and case management services Follows a Housing First model *Veterans interested in this program should have an assessment completed by HCHV at the Capitol Hill Outreach Center
Housing Progress HCHV Program completed 641 full assessments in fiscal year 2023 HCHV program has had nearly 2,500 clinical contacts through Outreach, Walk-In Clinic, and Case Management Services thus far in fiscal year 2023 Currently 306 Veterans are housed with HUD-VASH vouchers Another 53 Veterans are working towards permanent housing through the HUD-VASH program A total of 111 Veterans have been housed in fiscal year 2023 through HCHV services Of 138 Veterans housed through VA Homeless Programs this calendar year only 1 Veteran lost their housing.
