  2. Behavioral Health System of Care A framework for services and supports that are Person-Centered , Trauma Informed, and Designed to readily adjust to meet an individual s needs and their chosen pathway to recovery.

  3. Person-centered and Trauma Informed: Allows for individual choices that fit a person s needs throughout the recovery process and acknowledges that mental health and substance use disorders are chronic diseases not the result of a person s flawed character. Some Elements of a Recovery Oriented System of Care Engages a Support System: We believe in the importance of families and other allies as support systems for the person in recovery. Individualized and comprehensive services across the lifespan: Like any other chronic diseases, behavioral health diseases need individualized, comprehensive, and flexible treatment.

  4. Community-Based Services: Care is delivered in the community to enhance the availability and support from families, intimate social networks and peers. Some Elements of a System of Care Continued Continuity of Care : This includes a full continuum of care, including pretreatment, treatment, continuing care and recovery supports. Public Health Aligned: Community Behavioral Health aligns with and compliments the County Health Improvement Plan (CHIP)

  5. Strengths-Based : A system of care that emphasizes individual strengths, assets and resiliencies. Some Elements of a System of Care Continued Culturally Responsive : It is culturally sensitive, competent and responsive to all community members. Equitable access to services. Committed to peer recovery services : It includes peer recovery support services, individuals with lived experience. Integrated services :It encourages coordination and integration of services and supports to best meet the individual s unique strengths, desires and needs.

Intervention Screening-SBIRT Multiple Settings Including On-Line Anonymous Web- Based Screen Community Care Clinic-Walk-in Screening and Assessment Prison and Jail Re-entry (Linkages) Crisis Diversion Parenting and Family Member Education and Support (CRAFT) High Fidelity Wrap Around for Families and Multi- System Adults Outreach at Community Sites Client s Rights and Advocacy

Crisis Intervention, Mobile Crisis, and Mental Health Crisis Bed-Adults and Youth Medication Assisted Treatment (MOUD) Psychiatry Therapies cognitive behavioral therapies, motivational interviewing, trauma-informed care, CBI-CC, EMDR, DBT, Contingency Management, etc., including walk-in services Specialized Docket Courts Behavioral Health Navigator-Adult Probation Residential Treatment- Substance Use Disorders Mental Health Hospitalization State Hospital and Private Forensic Services in the Hospital and in the Community Medically Managed Withdrawal (Detox) Treatment

Recovery Supports Housing-Temporary Assistance and Permanent Vouchers for Mental Health Peer Support Mutual Aid Groups AA, NA, Smart Recovery Recovery Housing for SUD Welcome Home Packs for Prison Re-entry Job Readiness and Employment Supports Linkages to Health Care Services Transportation Emergency Assistance Guardianship/Payee Program Assisted Outpatient Treatment Mission Program Community Counseling Center- Club House

  11. Screening: A tool that can be administered by any trained person to determine if someone needs to have further mental health and/or substance use disorder assessments. Intake: An appointment to gather information needed to enroll the client into services but not an assessment. DEFINITIONS Assessment: Clinical evaluation conducted to determine a preliminary diagnosis and assists in developing a treatment plan. Level of Care: This can be a level of outpatient services or inpatient services. Level of Care is determined by the Assessment.
