Forms Report: Description & Intended Use Provides a list of all tagged forms on your site Delivers insight to help assess overall effectiveness of each form in driving visitor completions/submissions. For each form listed in the report, the number of sessions where the form was accessed by a visitor is displayed as well as the total number of page views for the form. The number of times each form was either submitted or unloaded is provided along with the amount of time visitors interacting with each form. Setup Form Action Tag: This tag is thrown automatically for forms on a web site as long as you are using the most current tagging library and pages where forms appear are tagged. IBM looks for HTML forms on a web site and populates form action tags for these forms Best Practices Tip: We advise our clients to use business/ user friendly names for form names and fields.   4. Common Form Analysis Scenarios How many visitors interacted with the form ? Can be determined using the Unloads metric in the Forms Report. Unloads indicates the total number of times a visitor interacted with a named form by editing a text field or dropdown, but then failed to submit the form. How many visitors completed filling out a form? Can be determined using the Submit metric in the Forms report. However, this metric is irrespective of whether the form submit was successful. How many visitors edited a form but did not complete it? Can be determined using Unload metric. See above. Where within each form are visitors abandoning the forms? Can be determined using the Form Zoom report Form Field Analysis. To identify areas of abandonment, determine which Form Field has the highest Unload %. Are my forms too complicated to fill out? Can be determined using Average Time metric in the Forms report. Evaluate forms with high Average Time to review design flaws that may be creating visitor confusion. IBM Corporation 2017 4

In this example, there is only one form on the example page. However, client s can have multiple forms on one page.
