O D - guidance All Wales Guidance from BHNOG First issued in January 2020, with letter from the CMO Revision in December 2021 KPIs for major haemorrhage incorporated Recommendations laid out with Actions and/or Measures
Issues & Wastage Monthly Preceding 12 months data shown Total 12-month data calculation All Wales, HB and Hospital level All HBs / Hospital shown to allow benchmarking
KPIs O D negative issues 12% of total red cell issues O D negative wastage 10% of O D negative issues O D positive is used as emergency red cells in adult males for management of major haemorrhage Pre-transfusion sample is taken, or valid group is available, prior to first unit of blood given Tranexamic Acid is given as part of the management of major haemorrhage in Trauma and Obstetrics Hospital transfusion laboratory team are informed that the major haemorrhage was stood down
KPI report Quarterly All 6 KPIs Submitted to BHNOG Sent to HTT/HTC for discussion
Gap Analysis & Action Plan Gap analysis of compliance with recommendations Action plan for partial or non- compliances Local data within O D- issues and wastage report, and KPI report aids analysis Returned to O D- working group for review
