BTC Care Management & Racial Equity
Bridge to Care's mission is to provide free care for patients with chronic diseases while offering support from a diverse team. This initiative works closely with patients to address barriers to care, manage communication, and connect individuals to essential resources. Moreover, the project emphasizes equality in healthcare by focusing on demographics, such as patients with hypertension, diabetes, and hyperlipidemia who face obstacles in accessing health services. The current progress includes strategies for graduate student recruitment, faculty guidance, community partnerships, and training sessions for care managers. Ongoing projects involve REI Groundwater Training and online initiatives for SHAC patient advisory board.

BTC & RACIAL EQUITY IN MEDICINE BTC Patient Demographics Seen > 1x in 2018: 40 patients Hypertension, diabetes, & hyperlipidemia Over 95% do not have health insurance >70% black and/or Hispanic patients People of color face increased barriers to accessing health care1 Prevalence of diabetes is higher in non-Hispanic black men and Hispanic men2 Rates of premature death (<75 YO) from stroke & coronary artery disease were higher in non-Hispanic black men & women than among white men & women. 2
