  2. Summary: This webinar is aimed at providing information about what is available in Buckinghamshire to support your child or young person s health needs and improving your confidence to make the most of it. Navigating the health service It is an update of our 2022 webinar.

Designated Clinical Officer (SEND) Supporting health professionals to contribute to the Education, Health and Care Plan process Working with others in the local area to improve the quality of Education, Health and Care Plans. Supporting the SEND team at the local authority with questions and queries Promoting links between the SEND team, education professionals, social care and health services Supporting commissioners and senior leaders with regards to meeting and assuring the SEND health requirements Working alongside commissioners and stakeholders including children and young people, parents and carers to identify and raise any commissioning gaps Working with commissioners to enable a good local offer of health services and clear health journeys for those age 0-25 with SEND 4

Mental health support and management of conditions like depression and anxiety. Minor surgical procedures such as removing moles or stitching minor wounds. Universal/Targeted Health visitors Your Health Visiting Service | Buckinghamshire | Health for Under5s You will have regular contact with your health visiting team from your baby s birth to your child reaching 2 years old including all the important milestones along the way. Referrals via gp Family nurse partnership Family nurse partnership - Buckinghamshire Healthcare NHS Trust - Birth Choices Website If you are having your first baby and are aged 24 and under, are less than 28 weeks pregnant and live in Buckinghamshire, you may be entitled to a Family Nurse. Responsibilities include: Health Assessments: Health Promotion: Vaccinations: Support for Medical Conditions: Emotional Well-being: Safeguarding Transition Support A parent, carer or healthcare professional. Conditions for the referral The child or young person: must be over aged 4 years 6 months and attending a mainstream school in Buckinghamshire can t have similar care and support for the same reason from another service. The referral must demonstrate: consistently implemented self help strategies with little positive sustained change. that the parent/carer has completed a relevant parenting programme and implemented the strategies with little positive sustained change. 14

Universal/Targeted/Specialist Home - Buckingham shire Healthcare NHS Trust - CYP Website Support for children aged 0 to 19: Support including fact sheets, advice sessions for parents/carers and professionals, webinars led by speech and language therapists, occupational therapists and physiotherapists is available without a referral form from the children and young people s therapies service. Children and young people who have high, urgent health or equipment needs require referral form.   16. Targeted/Specialist Buckinghamshire | Oxford Health CAMHS 1. Community CAMHS: Provides assessment and treatment for a variety of mental health issues. Crisis and Home Treatment Service (CAHTS): Offers urgent support and home-based treatment for young people in crisis. Eating Disorders Service: Specialized care for young people with eating disorders. Enhanced Case Management (ECM): Intensive support for those with complex needs. Forensic CAMHS: Services for young people involved with the criminal justice system. Learning Disabilities Service: Support for young people with learning disabilities and mental health needs. Mental Health Support Teams (MHST): Work in schools to provide early intervention and support. Outreach Service (OSCA): Provides support to young people in their communities. Inpatient Services: Includes units like the Highfield Unit for more intensive care. 10. Dynamic Support Facilitation Team 11. Thames Valley Link Programme To access Oxford Health CAMHS (Child and Adolescent Mental Health Services), you can follow these steps: 1. Self-Referral: If you are 12 years or older, you can self- refer using the online referral form available on the Oxford Health CAMHS website. 2. Professional Referral: If you are under 16, a professional such as your GP, school health nurse, teacher, social worker, or youth worker can refer you by contacting the CAMHS Single Point of Access. 3. Direct Contact: You can also contact CAMHS directly on 01865 901 951 4. Emergency Help: For urgent help, do not use the online referral form.   17. Specialist Community nurses for children and young people with a learning disability Work with you and your child/young person s behaviour, toileting, sleep and health promotion. The team can you give you advice and support if your child/young person has recently been diagnosed with a learning disability. Referrals Any professional who knows your family, for example a teacher, healthcare professional or social worker, can make a referral to us on your behalf. They will write an individual care plan with strategies to help you address their needs. Your child must be: aged 0 to 19 years and formally diagnosed as having a learning disability They also: work with other professionals to help enhance your child/young person s overall experience of our services be registered with a GP in Buckinghamshire. Our health visitors will assess and manage continence needs until your child s 5 years old. give your family information and advice, or signpost you to other resources and services We can work with children/young people in residential settings for no more than 38 weeks per year. support your child/young people and your family through transition into adult services work with groups of children /young people in schools around growing up and health promotion Download and complete the CNCLD referral form if you re a healthcare professional, social worker or teacher . The team works with children and young people in their own homes, schools and respite units according to their needs. 17

Specialist BHT Community Paediatrics See children with complex developmental needs or disabilities who have: Referrals can be made by a health professional or educational professional using forms on BHT Community Paediatrics significant difficulties at both home and school and have failed to respond to interventions put in place, so already have professional support in place. 18
