  1. CAPITAL AREA HUMAN SERVICES Mobile Crisis Outreach Services

  2. TEAM MAKEUP We are an interdisciplinary team of prescribers, social workers, peer support specialist, and case managers who help manage clients in crisis. We respond to crisis happening within our catchment area with the purpose of decreasing hospitalization rates while trying to ensure clients remain in their homes. After de-escalation and stabilization, we continue to follow up to ensure higher levels of care are not needed while referring to appropriate levels of care.

  3. WHERE WE SERVE

  4. CRISIS TEAM We have LCSW/Peer Support Specialist answering crisis phone and ready to respond to crisis within our catchment area. After assessment, we determine what is in client s best interest. Appropriate, community-based referrals made as determined. If staying within CAHSD continuum, referrals to outpatient clinics, community psychiatric support team, first episode psychosis, school-based, or other referrals as indicated. These clients are prioritized in our outpatient clinics in an effort to decrease likelihood of additional crisis. If outside of CAHSD, after referral, we don t leave the client. We ensure clients are indeed engaged in services.

  6. COMMUNITY PSYCHIATRIC SUPPORT TEAM (CPST) After crisis stabilization, referrals to CPST team are generally made assuming client is willing. Once referred, majority of services occur in clients homes. In-home evidenced based practices are implemented with stabilization and decreasing the need for higher levels of care as focus points. In addition to therapeutic intervention, wellness, recovery, needs assessment, psychoeducation, employment, behavior management, and increasing insight are some focus areas. We focus on the complete individual to ensure all needs are met. Lastly, we work with outpatient clinics to keep clients engaged with outpatient treatment requirements

  7. FIRST EPISODE PSYCHOSIS TEAM (FEP) CAHS FEP program assists individuals who are experiencing their first episode of psychosis. These individuals are identified through our crisis line or outpatient clinics and are then followed by this team. Team consists of Prescriber, LCSW, Family therapist, Peer Support Specialist, and Case Manager. Team works with clients and their families to deter crisis by increase treatment engagement and compliance.
