Preventing loneliness and ill health in later life Embedding understanding of the key transitions/risk points Evidence base/vision/calls to action What offers do we have to support people experiencing these transitions? Where are the gaps/opportunities? Bringing it together pledging for action Our research shows that life transitions are key triggers for loneliness. We need to focus on these moments and work together to prevent loneliness from taking hold in the first place

Attendance and operational effectiveness 110 attendances booking to attendance ratio = 72% Over 95 organisations represented 28 new teams/organisations linked to the Programme Operational effectiveness remains strong: Range of services/organisations, facilitation & organisation, welcome and accessibility, structure and time keeping 4.7/5 (94%) Understanding of role in prevention loneliness 4.6/5 (92%) Caf discussions 4.5/5 (90%) New format well received: Like the breakfast idea, much better due to time pressure Very good and helpful meeting, really enjoyed and learnt from the cafe style - moving from one topic to another, great way to interact with other organisations and discussing different topics

Most of working age. 80% lonely/isolated. Little known about needs at key transition points 75% didn t get support needed. Increases risk of mortality/ poor health. Annual cost of hospital stay between 150 and 190 million Delaying the onset of one LTHC across the population by one year = annual saving to the NHS of 30.8 million Only half of workers looking forward to retiring. Many don t plan or receive guidance /support Context/ Impact Carers supported and recognised as individuals, with needs of their own.
