CMC Recovery Center - Providing Comprehensive Care for Community Members
The CMC Recovery Center, implemented by Community Medical Centers in January 2018, is a grant-funded program offering whole-person care for individuals with histories of homelessness, substance use, behavioral health issues, and interactions with law enforcement. The program aims to address the needs of mild to moderate populations, reduce recidivism rates, and extend services to additional locations like Tracy Central. With a dedicated Recovery Team offering various services, including medication-assisted treatment and group counseling, the center focuses on holistic recovery. Additionally, the Hub and Spoke System expands services to various facilities and highlights ongoing growth in the team. Respite and withdrawal services are also provided, offering a supportive environment for individuals undergoing recovery. CMC, Recovery Center Jan. 2018 Community Medical Centers implemented CMC, Recovery Center is a grant funded program through County Behavior Health with Prop 47 and MHSA Innovation funding to provide a Team Whole Person Care approach to individuals in the community, that may have history of Homelessness, Substance Use, Mild/Moderate BH and/or history with Law Enforcement. Program History & Information Program is to help meet the needs of MILD/MODERATE population for BH, Substance Use and to decrease the recidivism in the jails and ER s. Recently able to extend services into Tracy Central site through support from HRSA.

Recovery Center Respite & Withdrawal Services: 7-14 day stay -TOC 15 Bed Dormitory Style *9-Male, 6-Female 2 Private Rms for Special needs Laundry, Meals & 24 hr. monitor MAT Services Counseling- Individual & Group Case Management Life Skills Classes/Activities Community Programs Presentations By 2020, possible location by CMC Channel Clinic
Recovery Respite Center Services o 15 Bed (12 dormitory and 1 private rms) Short term Respite Facility for 7 to 14 day stay during transition to long term placement. o Medical clinic within the site to provide MAT and/or Medical Triage Services o Vivitrol Injection Clinic o Case Management with strong emphasis on transition and stabilization. o SUD and Behavioral Health 1;1 and Group Counseling o Harm Reduction Focus o Life Skill Programs/Classes from Community Partners Future Plans
The Recovery Center has seen a rise in referrals from initial start year 2018 of 186 referrals with 96 of those enrolling in Recovery Center Program to a current total of 2223 referrals with 937 enrolled in program, estimated 55% of these referrals are made internally Patient Referrals and Enrollment
The numbers of participation in these areas has also grown from the initial year 2018 of 96 enrolled members to current 937 enroll members. These numbers are only a snapshot of the impact the Recovery Team and program has made on the community, in the short few years of operation. Patient Referrals and Enrollment
CMC, Recovery Center continues to grow as it moves to meet the needs of the community: Completion of Respite Center (licensing and operations) Implement Vivitrol Injection Clinic at Respite Medical Center Automatic Electronic Data Reports for program reporting 24 hour Sobering Service Continue strengthen the Adolescent and Youth SUD program w/MAT services Future Plans
Referral Criteria: Individuals with Substance (any type of drugs and/or alcohol) use challenges Homeless Mild/moderate mental health challenges. History with Law Enforcement Uninsured and Underinsured Curious individuals with questions about the program. NO wrong door philosophy Referral Process: Who can be Referred? How? 1. Community Partners- Fill out Referral Form & email/fax to Recovery Center or provide phone number below for self referral.(Forms are provided at request. Client made aware of referral and agree to being contacted). 2. Self Refer- 1-(209)-940-5662 providing patient with Recovery Center phone number to call and set up appointment or ask questions. Voice Mail is available and checked daily. Patient is outreached within 48hours to set up appointment. Transportation assistance is provided as needed.
