US ARMY COMBAT READINESS CENTER READINESS THROUGH SAFETY Mishap Synopsis Mission: The Soldier was the single occupant of a M1097A1 HMMWV conducting movement to a training area. While driving on a civilian highway in route to a training area, the driver failed to yield the right of way when he turned left into the path of an oncoming motorcycle. The civilian motorcycle rider was unable to stop and collided with the right side of the HMMWV. The civilian motorcycle rider was ejected from the motorcycle and struck the side of the HMMWV. The HMMWV sustained significant damage, the motorcycle was destroyed and the motorcycle rider was fatally injured. US ARMY COMBAT READINESS CENTER READINESS THROUGH SAFETY Key Facts The mission was routine and repetitive. The task force conducted deliberate risk assessments for the mission. However, the unit did not address the hazards of driving off the installation. A proper PMCS was not conducted when the vehicle was dispatched and the driver was not listed as an operator. Speed was not a factor for either vehicle. The road was unlit, and while fog was present, the headlights of oncoming traffic were visible.
