  2. Why we care about digital inclusion We support digital inclusion because digital is an enabler. Digital inclusion is social inclusion. Everyone should be able to make an informed choice.This means: Understanding the personal relevance of digital to their lives and how using digital tools, technology, and services can benefit them. Feeling confident in their ability to engage with digital. Having access to the resources they need to get connected. Having access to the resources they need to be able to develop their digital skills. Those who would most benefit tend to be those who are least likely to be online. Everyone in Leeds should have equal opportunity to use digital tools, technology, and services in the right way for them.

  3. Building cross-sector capacity to offer digital inclusion support. Supporting digital inclusion for improved health participation. Supporting the rollout of digital tools and services. Digital inclusion awareness training for staff and volunteers. We want everyone to be able to: Use the right digital tools and services for them. Use the right health services at the right time. Be able to make the right (informed) choice for them. Understanding how digital exclusion might impact on programmes and minimising risk.

Taking a community-based approach Strengthening the place to empower the person Designed by experts to meet the specific needs of a community Embedded within the services people already use Embedded into existing person-centred holistic interventions Understanding complex need and able to assess risk Utilising existing trusted relationships and specialist training Community Working in partnership to share learning and resource, and maximise impact Digital as an enabler, addressed in the context of individual motivators Ongoing and sustainable

  5. Developing Digital Health Hubs Supporting digital inclusion for improved health outcomes Trusted places & people Person centred, holistic, adapting to changing needs Digital inclusion support Digital health support Digital Health Hub

Collaborative and supportive Digital Health Hubs Responsive to community need A network of trusted community organisations embedding digital engagement and support within their offers, and championing the benefits of being online to support increased health participation. Everyone has a role to play Embedded within own work, policies and agendas

  7. Digital Health Hubs support digital skills. Digital Health Hubs provide connectivity. Digital Health Hubs provide equipment. One-to-one support. Group sessions. Home visits. Laptops and tablets available to use on site. Tablets available to borrow and use at home. Free wifi on site. Gifting 4G data, calls, and texts.

  9. The NHS app has saved me time when booking doctor's appointments for my wife, and with the help of Hamara I found a job as well. Being able to get online has reduced my stress and helped me to access the information I need. Supporting digital access and skills in a person-centred way. Having person-centred motivational conversations about how using particular digital tools and services could make someone's life easier. Letting us know about changes and innovations with a digital element so that we can make sure Digital Health Hubs have what they need. Telling people about their local Digital Health Hub and what support is available.
