What should be reported in IRIS? Any occurrence, event, or happening that: Is inconsistent with the routine care of a patient Is inconsistent with normal hospital or clinic operations Adversely affects patients, physicians, employees, contract personnel, volunteers or visitors That could have or did cause harm or undesirable impact (including near misses) Urgent reporting concerns: Events with high severity or urgency, or events with the potential to cause harm to other patients Contact a site-based Risk Manager to report the incident AND complete an IRIS report! / Physician Orientation - IRIS 3
Why complete an IRIS report? Identify trends and opportunities for growth Improve performance Enhance a culture of safety for patients, staff and visitors Promote quality of care To ensure a highly reliable organization To put Safety First! Every incident, regardless of the outcome, is read and investigated. / Physician Orientation - IRIS 4
When should an IRIS be completed? Complete an IRIS report As soon as possible! On the same shift in which the event or incident occurs and No later than 24 hours following the event IRIS report completion is neither documented nor referred to in the medical record! This includes any documentation of the facts, any follow- up treatment required, communication with the patient or their representative, and communication of any hand off that occurs as a result of the event. Document as usual, without any mention of an incident report being completed. / Physician Orientation - IRIS 5

Another Way to Submit an IRIS For Providers without access to a computer: Leave a voice mail on the Patient Safety Hotline: Patient Safety Hotline: 253-426-6289 (or X127-6289) Provide complete details of the incident Be sure to include your contact number so you can be reached to verify information The Patient Safety Hotline is CONDIFENTIAL and is checked each business day OR, contact a Risk Manager directly to assist with incident report entry / Physician Orientation - IRIS 8
IRIS is a Patient Safety Work Product Privilege An IRIS Report: Is a protected, Quality Assurance document, when embedded within the IRIS system Is a confidential report, which may contain protected health information Is not a public record and shall be used only for purposes as provided by law REMEMBER!
