  2. IMPORTANT REMINDERS The application is a fillable PDF Do not print out and scan your application There is a character limit Don t forget to sign it Housing Action Illinois will review your application if we receive it by March 12, 2018

  3. Applicant Information (P. 4) If your HUD certification expires during the grant reviewing period, you must get an extension letter from HUD

  4. Experiential Information (P. 5) Client-related Activity This is pulled directly from the HUD 9902 form Include your 2016, 2017, and 2018 HUD 9902 forms For 2016 and 2017: use Q4 only For 2018: use Q1 Grant Request Information Requested Grant Amount this amount must match the total amount in the budget document on page 10 (Proposed Plan)

  5. Organization Capacity (P. 5-7) Mission and vision statements Copy and paste If you do not have a vision statement, describe it as best you can Question 2A All active funds Include FPP-G 1 funding in this section Question 2B Closed out funds not a repetition of question 2A

  6. Organization Capacity (P. 5-7) Question 3 Indicate all active marketing efforts you are currently doing Do you have any plans for improvement? If you are not tracking success, indicate how you will under FPP-G 2 Question 4 Group your outreach efforts together at the top, then group regular classes together toward the bottom of your list

  7. Program Need and Demand (P. 8) Population U.S. Census website Foreclosures filed County data, Realty Track Unemployment Rate IDES website Layoffs WARN Act, Illinois Department of Commerce and Economic Opportunity Home Buying trends Illinois Association of Realtors

  8. Team Capacity and Experience (P. 9) Answer all three questions in response to the figures you provided above in question 5 Question 6 If you can, brag about your organization s work thus far Question 8 Explain this well, using the data you found for question 5

  9. Housing Counselor Certifications(P. 11) Don t forget to attach all certifications for each counselor!

  10. Proposed Plan (P. 10) Don t forget! Computer & Equipment purchases, General Operation, and Technical assistance combined cannot be more than 10% of the total grant award Don t forget! The total amount must match the amount in Total Grant Request on page 5

  11. Audit Information (P. 1) Financial Audit Include your entire audit and letter from your auditer HUD Audit Include everything that was provided to you by HUD

  12. Future Plans (P. 2) This section is for the future talk about your plans if you receive FPP-G 2, not your current year plans for FPP-G 1 Question 20 Be thorough, and remember there is a character limit Question 21 If you do not currently have a strategic plan in place to address barriers to providing services to individuals, you may write down that you will work with Timm Krueger, Housing Action Illinois, to identify those barriers and explore options Question 22 Keep your answer to 3-5 sentences; be specific

  13. Exhibit Checklist (P. 3) Review the checklist thoroughly Double check everything! Don t forget Housing Action Illinois will review your application if you send it to us for review by EOD Monday, March 12, 2018
