Types of Cases Positive COVID-19 (asymptomatic or symptomatic) Symptomatic, but Negative COVID-19 test Exposure at work or outside of work 4
How do I generate a claim? What do I do now? 5
Positive COVID-19 Test Asymptomatic or symptomatic, if you test positive for a COVID-19 test, you are eligible for a leave of absence. False negatives also warrant a leave of absence (but the false negative must be certified by the clinic). 1) Can you work from home? 1) If yes, then, you follow the instructions of the doctor and keep working from home. 2) If you cannot work from home, you need to file a claim with Lincoln. 6
How to File a Claim with Lincoln 1) Call Lincoln Financial Group at 888.244.7535. 1) Enter your social security number and press option 1 for each prompt. 2) For your call, you ll need to provide the following information: 1) Date you last worked a full day. 2) Date of first symptoms or told you had an exposure. 3) Date of test. 4) Date diagnosed. 5) Potential return to work date. 6) Bank account information: routing and checking account numbers. 7) Doctor s information: Kathryn Gibson, phone 801.584.5144, fax 584.5206 7
Symptomatic, but Negative COVID-19 Test If you are showing symptoms and told to quarantine, and then, receive negative results for your COVID-19 test, you can apply for a GLTS (Grandfathered Long-Term Sick) request. Per company policy: Use 2 days of PTO, and then, GLTS. If you don t have PTO, then, the first 2 days are unpaid. Paid Out by HR. Exposure at Work or Outside of Work For any type of exposures, there are four scenarios: If your test comes back positive: Follow the procedures previously listed in slide 6. If your test comes back negative, but you are still required to quarantine, and you are asymptomatic: You must use PTO or designated pay by the company. If your test comes back negative, but you are symptomatic and required to quarantine: Follow the procedures listed in slide 8. If you are not tested and required to quarantine, per the clinic: You must use PTO or designated pay by the company. 9
Pay Options Positive COVID-19 (asymptomatic or symptomatic) You may be eligible for short-term disability pay through Lincoln, Pandemic Pay, GLTS, and/or PTO. Symptomatic, but Negative COVID-19 test You are eligible for pay through Pandemic Pay and/or a GLTS Request. Exposure at work or outside of work You are eligible for pay through Pandemic Pay and/or PTO. Some cases may be eligible for unemployment through Dept of WFS. 11
