  2. COVID-19 Provider Call Center Schedule Update Tuesday, December 6 the COVID-19 Provider Call Center is closed for an All-staff Meeting. The COVID-19 Provider Call Center will resume its regularly scheduled hours on Wednesday, December 7, 2022. 2

  5. myCAvax and My Turn System Outage To complete deployment of Release 33, both myCAvax and My Turn will be unavailable from Friday, December 9, 2022, at 6:00PM PT until Monday, December 12, 2022, at 8:00AM PT. 5

Account Changes for My Turn Users with the Unified Login Experience Beginning Monday, December 12, 2022, My Turn only users with a username ending in .myturn will receive a new username ending in .mycavax. Your password will not change. Your old login will no longer work. If you currently access both My Turn and myCAvax, beginning Monday, December 12, 2022, you can use the same login credential, ending in .mycavax, to access both systems. 6

  7. Single Dose Pfizer (12+ Years) Bivalent Ordering The new Single Dose Pfizer (12+ years) Bivalent product is available for ordering to Providers. To stretch limited supply, the maximum order size is 50 doses. Third Party Redistributor small orders are expected to be available within the next 2-3 weeks. Due to limited supply, MCEs have not yet received allocations for this product. Additional allocations are not expected until after the first of the year. 7

Pfizer Single-Dose Vials National Drug Code (NDC): 59267-1404-02 Configuration: 10 single-dose vials/carton Product dimensions: 1.457 in length 1.535 in width 3.504 in height Storage and handling: Same as other Pfizer tris products (e.g., store at ultra-low temperature until expiry, may refrigerate up to 10 weeks within expiry period) No ancillary kits included with single-dose vial orders Single-dose vials do not require the use of low-dead-volume (LDV) syringes

Anticipated Bivalent Vaccine Expansion for Additional Pediatric Groups* Pfizer bivalent vaccine: For 6 months through 4 years as a third dose for primary series completion Those who already received 3 monovalent Pfizer doses not expected to be authorized to receive the bivalent product Moderna bivalent vaccine: for 6 months through 5 years as a booster dose Not anticipated to have mix-and-match authorization CDC anticipated to provide recommendation in mid- December, pending FDA authorization *Preliminary data that may be subject to change CDC Fall Vaccination Operational Planning Guide 9

Announcement: Infant/Toddler Bivalent Vaccine Pre-Ordering CDC anticipates EUA issuance and a recommendation for Pfizer and Moderna bivalent COVID-19 vaccine for infant/toddler age groups in early to mid- December. CDC has provided allocations for pre-orders, allowing vaccines to ship immediately following EUA. Pre-ordering information: myCAvax pre-ordering window for LHJs/MCEs/Providers is now open through Friday, December 2 by 5PM. Pre-orders will be transmitted to CDC on Monday, December 5th (one wave) Minimum order: 100 doses 10

  12. Infant/Toddler Bivalent Vaccine Pre-Ordering Additional Pre-ordering information: Doses that are pre-ordered will begin being processed for delivery following EUA. Expected delivery information is not yet available as it will depend on the EUA. Routine ordering for the new products and the small orders timeline [Pfizer only through the TPR (AmerisourceBergen)] will be shared once we receive more information. The regular order cadence will continue for existing approved COVID-19 vaccine products. Infant/Toddler Bivalent Vaccine Product Information Pfizer* Ultra-cold freezer storage (-90 C to -60 C) until expiry NO FREEZER STORAGE (-25 C to -15 C) Refrigerate (2 C to 8 C) up to 10 weeks without puncturing Requires 2.2 mL diluent per vial (providers will NOT be able to opt out of receiving ancillary kits) Packaged in 10-dose vials in cartons of 10 vials each (100 doses total) Dose 3mcg/0.2mL The minimum quantity for pre-orders is 100 doses Maroon cap identical to the monovalent Pfizer-BioNTech product for this age group New label identifying the product as a bivalent vaccine (i.e., Original and Omicron BA.4/BA.5) Once punctured, each vial must be used within 12 hours * Anticipated
