CYCORD CBB Training for Cord Blood Collection Sites 17 September 2021
General Information 2005 Establishment of CYCORD Public Allogeneic Cord Blood Bank (CBB). Applications from maternal donors for public bank storage and listing in Cyprus Bone Marrow Donor & Cord Blood Registry for unrelated Bone Marrow Transplant (BMT). 2016 Received Fact NetCord Accreditation 2020 During covid-19 pandemic, no collections were performed At end of 2020, the Ministry of Health and the CYCORD CBB decide to stop accepting new unrelated maternal applications. All unrelated units stored continue to be listed in the WMDA Search and Match. 2021 onwards Only applications for directed donation are accepted (medical indication case-by-case).
What the Karaiskakio Foundation Provides 1. Training material (Application form for Directed Donation which includes the Informed Consent and the Medical questionnaire) for verifying informed consent of the maternal donor 2. Written instructions (WI-SOP-06) for the method of collecting cord blood. 3. CYCORD cool box kit with all necessary means (including consumables)
Doctor Responsibilities 1. Maternal donor is informed and counselled by the physician who recommends banking the infant donor s cord blood. The intended recipient should be specified in the application form. 2. The maternal donor must consult her obstetrician to complete the Application Form. 3. The obstetrician will have to sign an agreement with CYCORD and will receive the training material. 4. The obstetrician will have to explain the information to be provided to the maternal donor, answer any questions, ensure their understanding verify that the application form is completed correctly, and that consent is signed after all information was provided. 5. Consents forms must also be signed by the obstetrician undertaking the delivery and consequently the collection of the cord blood unit agreeing to perform the collection in the best way possible to achieve its successful listing in CYCORD.
Doctor Responsibilities 1. The Doctor/Obstetrician has the responsibility to decide whether the collection procedure will be performed depending on the medical condition of the mother and infant and in any case the Doctor/Obstetrician shall not attempt cord blood collection if he/she believes it is contraindicated. 2. The Doctor/Obstetrician shall inform the CBB of any adverse reactions that might arise during the collection of cord blood, through the Delivery and Collection Form. 3. The Doctor/Obstetrician agrees to complete and sign all the necessary documentation concerning the collection procedure. 4. The Doctor/Obstetrician offers this service pro bono.
Instructions for Cord Blood Collection In-Utero Note: Once you receive the cool box, open it, remove the ice pack and place in the freezer. You will need to place the ice pack back into the cool box once the collection is completed. A. IMPORTANT: PRIOR TO COLLECTION There shall be no alteration in the standard medical procedures for the management of the labour to accommodate for a difficult or problematic cord blood collection. The collection shall not be attempted in cases where the attending clinician believes it is contraindicated (this may include premature births and cases such as nuchal cord or maternal haemorrhage). Ensure and verify that consent for the collection has been granted. Ensure that the collection kit belongs to the mother by verifying mother name, mother personal ID# on label affixed on cool box. Note if any antimicrobial treatment has been prescribed in the last 7 days. Indicate whether the mother is healthy on her delivery date. If mother has any health problems, please record this on the document provided.
In the CYCORD cool box, you will find the following: A sterile, single-use cord blood collection bag, packed in plastic package. A set of five (5) pre-labelled maternal blood sample tubes, in a plastic bag. The YELLOW Delivery and Collection form, to be completed after the Cord Blood collection. This must be completed and signed by (a) the paediatrician present during the delivery and physician performing the collection who has signed an agreement with CYCORD documenting training. An ice-pack (please place the ice-pack back to the cool box). An insulating separator for the ice-pack. One zip locked bag, pre-labelled with mother s name and KF code. A Max/Min electronic temperature recorder. An ISBT LB03 label to affix on the collection bag post-collection. Security tape for the cool box.
Ensure that all items included in the collection kit belong to the mother by verifying mother s name and personal ID#. Ensure that all labels include KFTCBU-## and ISBT LB03 labels. Collect the maternal samples during the preparation, or after delivery (2x4ml EDTA tubes and 3x4ml red-top tubes). IMPORTANT NOTICE: In case of any fluid infusion (blood products, saline etc) to the mother greater, than 1500ml 3 hours prior to sample collection, the blood samples should not be collected. Any fluid infusion 48 hours prior to collection should be reported. Make sure that all the tubes are labelled with mother s name, KF-TCBU-### and ISBT LB03 label. Once samples are collected, place them into the small zip-locked plastic bag.
B. CORD BLOOD COLLECTION PROCEDURE Note: In the CYCORD cool box and you will find a collection bag. DO NOT USE this collection bag if it has been damaged, if the anticoagulant is not in a liquid form, or if the collection bag is expired. ALWAYS wear gloves. 1. When the delivery procedure is completed and before the placenta is expelled, clamp the umbilical cord to avoid blood loss. 2. Open the plastic pouch of the collection bag. 3. Sterilize the section of the umbilical cord from which the collection will be performed. (Use alcohol. Do not use other antiseptic liquid e.g. betadine) 4. Aseptically vein puncture the larger vein with one of the two available needles in the collection bag. 5. When the blood flow stops, close the tube using the collection bag s clamp. Remove the first needle from the umbilical cord and cover. 6. If needed, perform vein puncture to the second vein, using the second needle in the collection bag, following the same procedure. (Never use the same needle for more than one vein punctures).
C. Post Cord Blood Collection Place the collection bag, the maternal samples and the completed and signed Delivery & Collection Form in the CYCORD cool box. Switch on the electronic temperature recorder. Before handing over of the cool box to the courier, ensure the following: You have placed the frozen ice pack in the cool box. The insulating separator is between the icepack and the collection bag. The maternal samples and the documents are on top, in the cool box. You have switched on the electronic temperature recorder. IMPORTANT: Seal the cool box with the provided security tape.
Place the sealed cool box in the dedicated, secured, access area of the collection site where entry is allowed only for authorized personnel. Contact the KARAISKAKIO FOUNDATION as soon as possible for pickup. When the courier arrives at the collection site for pickup, remove the sealed cool box from dedicated storage and hand over to the courier after completing and signing the courier s pickup/delivery form. ADVERSE REACTIONS In the event of an adverse reaction during the collection, attributed or suspected to be attributed to the collection procedure, please report to CBB.
