Historical Background of Door County Medical Center Partnership with Schools Partnership with Schools Exceptionally trained, highly professional therapists, athletic trainers, school nurses and physicians Rehab Services Staff at Gibraltar: Chelsey Veldboom, Occupational Therapist Kalla Linzmeier, Occupational Therapy Assistant Bill Herbst, Physical Therapist and Athletic Trainer Jordan Dielmann, Athletic Trainer (backup Tia Jackson) Excellent working relationships with coaches, teachers, athletic and special education directors, principals, etc. Quarterly Supt./DCMC Meetings

Current Purchased Services In-School Therapy: PT and OT Occupational and Physical Therapy Services for Referred Students for Past 40 Years Services Provided Up to 5 Days/Week Evaluation and Treatment of Students On-Site Coordination Between Therapists, Pupil Services, Special Education Directors, and Staff Attendance and Participation in IEP Meetings Ongoing Therapist Training Specific to Autism, Learning Disabilities, School Therapy, etc. Education of District Staff to Enhance Student Programs and Delivery of Services

  15. How Are We Doing? Survey Results of DCMC Staff Survey % Recommend # of Replies May 2019 100% 14 Dec 2020 100% 41 I love working with the school nurse, OT and PT staff at my school. They are definitely part of the team and put the student first. I am very grateful for having such a strong, professional team working with our students and staff. The DCMC school-related services and staff are excellent. When there are obstacles, the DCMC staff have proven to be flexible and responsive. ___ & ____are both amazing to work with. Their attention to the needs of the students and staff is just tremendous. We are so fortunate to have them working with us. ____ is an awesome school nurse. She is always very caring and patient with our students. ____and all the staff does a great job.

Commitment to the Schools and Their Communities Community Health Needs Assessment & Implementation Plan As part of Affordable Care Act, we partner with Door County Public Health and other organizations to improve the health of the community DCMC and Prevea are committed to partnering with the school districts to conduct health risk assessments, screenings, health education program, and efforts to increase physical activity, fitness, and wellness. Haven t raised School Nursing Rate in years: Increasing from $33 to $40 per hr. (our cost is $51.15 per hr.) with an annual auto renewal increase of 5% yearly. We will subsidize the difference in respect of district budgets.

Summary of New Consolidated Contract for County Schools One Master Contract Promoting Equity of Services Between and With Schools Continuum of Care for Students Who Transfer to Other County Schools Efficiency of Collaboration and Communication Between Districts and Hospital by Identifying Common Needs, Procedures, and Goals Same Schedule for Each District (fiscal year of July 1 June 30) Auto Renews Each Year
