Background Open disclosure describes the way staff communicate with patients who have been involved in a patient safety incident during their health care patient safety incidents include harm, suspected harm, no harm and near miss incidents. Staff at Sligo University Hospital (SUH) are working with the National Open Disclosure Team to develop a revised face to face training module for Open Disclosure. Currently there are two e-learning modules available on HSeLanD which provide an overview on how to communicate in an open, timely, empathic, and compassionate way with patients and their families when things go wrong during their health care journey. These modules have proved very effective in supporting staff through the Open Disclosure process. However, the need for additional face to face skills training for staff to practice open disclosure in a safe supportive environment is identified as an additional training need by the National Team, particularly for those staff who may have to participate in formal Open Disclosure meetings. Aim To build the capacity of attendees to prepare for and manage open disclosure meetings with patients, services users and their relevant person following a patient safety incident . This is a follow on module designed to support and complement Open Disclosure E- learning Modules 1 and 2. Change initiative Audit and training needs analysis identified the need Pilot team established facilitated by national open disclosure team Discussion and brainstorming exercise conducted by project team to decide on content A 2.5 hour workshop was designed incorporating managing Low Level Open Disclosure using the ASSIST Model of Communication, managing emotions & managing formal Open Disclosure scenario based approach using role play Rolled out to 36 healthcare workers including 13 Consultants across seven specialties. Each session evaluated. Formal meeting of project group and training participants to provide feedback A revised Train the Trainer programme was facilitated by national open disclosure team

Open Disclosure : What we would expect for ourselves or for a loved one. Open Disclosure: The professional, ethical and humane response. Open Disclosure: The empathic response to all those involved in and/or affected by patient safety incidents . Spread This training is suitable for all healthcare workers and particularly for all staff who may have to participate in formal Open Disclosure meetings. Additional information A. Policy: It is the policy of the HSE that staff will: Communicate with patients in an open, honest, transparent and empathic manner following patient safety incidents. Provide patients with a sincere and meaningful apology when they are harmed as a result of a patient safety incident. Begin the communication process within 24 48 hours of the incident occurring or becoming known to the health services provider. B. Legislation: The pending Patient Safety Bill makes provision for mandatory open disclosure of a number of Notifiable Patient Safety incidents. C. Training: Open Disclosure Training is mandatory for all staff with refresher training every three years. This training has been awarded 3 external CPD points by RCPI and 3 CEUs by NMBI
