Risk and Opportunity Management System - SQRA A hierarchical system focusing on identifying major hazards. Examples of these are: Single or multiple fatality(s) Environmental impact requiring 10 or more years to recover Business loss exceeding AUD$50m Mobile Plant and Equipment

Risk and Opportunity Management System QRA/JSA QRA Operational risk tool used for small projects or major activities Where an activity comprises of many tasks Identifies hazards and controls to complete the activity safety JSA Used for individual tasks within an activity Uses SQRA data Mobile Plant and Equipment

Hazard Identification The most critical part of any risk assessment is the accurate identification of hazards and their root cause. Risk tools should only be used by: - Trained, competent people - People who have experience in the area the risk assessment is being done Risk and opportunity management is only effective when controls are physically implemented for each hazard. Risk Evaluation and Escalation The 5x5 Risk Matrix is used when doing a QRA or JSA to determine whether risks are acceptable or not. A Hazard Risk Index (HRI) score is assigned Note: Risks with an extreme HRI score must be escalated to the General Manager and the Commercial team. OTML has two levels of risk registers: - Major Hazard Register - Local Risk Register Mobile Plant and Equipment

Effectiveness and Assurance Periodic audits and assessment programs, in conjunction with independent review by the Commercial team, facilitates continuous improvement. Risk reduction actions are common to the risk and opportunity management process. Actions are assigned to a responsible person Actions must be completed (closed out) within a specified timeframe All personnel involved with the risk assessment process are required to have training that s appropriate to their level of responsibility.
