  2. Confidentiality You may email the SOS Log. We recommend that you do not use a flash drive to carry or transfer info. If needed, save to your laptop and delete from the flash drive. Use LAUSD laptop/computers when completing all FYAP forms and SOS. Do not leave records unprotected, where they can be accessed. Check with an administrator if unsure of releasing student information. It is best practice to protect information, rather than risk violating student s rights and privacy.

Out-Of-Home Care Placement Types Type of Out-Of-Home Placement Definition 1 Adoptive Home Youth is living with a family who is in the process of adopting the youth. 2 Court Specified Home Rarely used, and it is when the court orders the youth to be placed with a specific family. 3 4 5 Foster Family Agencies (FFA) Foster Family Home Group Home Certified foster parents under the license of the FFA. Foster home licensed by the state. A large institution type home. 6 7 8 9 10 11 Guardian Home In Home Independent Living Placement Non-Foster Relative Home Small Family Home Youth is being cared for by their legal guardian. Child is living with their biological family. Youth 18 years or older. Youth is not in foster placement. Living with a relative. Can house up to six youth, and are usually utilized for our medically fragile youth.

  10. Common Mistakes E-mail: SOS logs not attached. SOS file: Does not have FYAP Counselor s Name. SOS file: Monthly worksheet empty. Caseload tab: Missing Student ID # and SSID #. Caseload tab: Start date incorrect. Caseload tab: Using the N/A option appropriately Monthly tab: Not entering a number or leaving data box empty. Monthly tab: Target and Universal Services: Only one column has data. Monthly tab: Notes do not match data columns. Monthly tab: Data entry for leads, PSA PD, and ESC meetings. Monthly tab: Using one row for the whole month. Total tab: Tally worksheet missing FYAP Counselor s Name. Total tab: New FYAP counselors claiming completed assessment from previous counselors.
