Community Model Hospital Reception Site Planning Guide. o Job Action Sheets. o Work Flows. o Checklists.
Activation System-wide Patient Family Assistance Plan. Activation Criteria: o Mass Casualty Incident response/recovery. o Any time deemed necessary by Healthcare Incident Management Team (HIMT) Incident Commander (IC). Operations Section Chief activates: o Patient Family Assistance Branch Director, who activates: Family Reunification Unit Leader (as necessary). Social Services Unit Leader (as necessary).
Space and Staffing Hospital Reception Site (HRS). o Primary and secondary locations identified. o Pediatric Safe Zone for uninjured, unaccompanied minors. o Planning considerations for access and functional needs. Staffing. o Registration personnel. o Child life specialists (or other appropriate staff). o Interpreters. o Security. o Crisis Support Staff (i.e. Chaplaincy, Social Workers, Critical Incident Stress Management, Banner Health CANDOR/Talk2Me team).
Banner Health Policies Health Insurance Portability and Accountability Act (HIPAA) o BH personnel may disclose PHI to a public or private entity that is authorized by law or its charter to assist in disaster relief efforts, in order to inform family member or other persons of an Individual s location or condition. (Policy # 1333). o In the case of mass casualty events, the hospital may provide limited information to an authority agency (i.e., law enforcement or Red Cross) for disaster relief for the purposes of notifying family members or significant others of the individuals locations and general condition. (Policy # 393).
