Certification Form Vendor will provide the below services (check all that apply): Supports related to COVID-19 risk management Completion of individual assessments and/or program plans Completion of a person-centered plan Remote services delivered via telephone or video communication Delivery of supplies and other items to the consumer s home Confirmed use of self-guided materials Services provided in-person* at the consumer s home Services provided in-person* at a community setting Services provided in-person* at the provider s facility Supports for transition to the Self-Determination program, if applicable Other (eg. Employment Discovery): _________________________________

  3. Certifications A COVID-19 Protection Plan has been developed and is being implemented to ensure participant safety for in- person services and includes training for staff and consumers. A discussion with every consumer is being coordinated to identify the service(s) that best meet their needs and interests. Regional Center Service Coordinators, residential staff, and parents/guardians are being consulted as needed. Phone and email access will be maintained during the following business hours to respond to needs from consumers and their families: Documentation of alternative services provided for each person will be maintained per Title 17, CCR section 54326. Documentation of staff training, including training attendance logs and curriculum, will be maintained. Topics may include, but are not limited to the following topics: COVID-19 safety precautions Person-centered thinking and planning Informed decision-making Self-Determination Program Use of plain language for written materials Alternative communication methods Cultural and linguistic competencies Use of technology

  4. Monthly Reporting (New) By November 30, 2020, and monthly after, the vendor shall submit to the regional center a report in writing of the alternative services delivered. Monthly report shall include: The number of consumers with service authorizations; The number of consumers served by the vendor who received alternative delivery of nonresidential services; The number of consumers with service authorizations who did not receive alternative delivery of nonresidential services; The number of consumers who used each type of Alternative Service A description of the alternative delivery of services provided to consumers that is not listed in subdivision (c), if any, and the number of consumers who used them; and The number of days in each month each consumer received an alternative delivery of services.   5. Provider Claiming (New) Information is pending from DDS on how these rates will be converted and invoices & purchases changed Vendors shall submit claims for alternative delivery of nonresidential services utilizing a monthly unit rate, calculated using the average monthly amount reimbursed based on the prior 12 months ending February 2020. For those 12 months, only those months where the amount of reimbursement to the vendor is greater than zero shall be included in determining the average.
