Paramedics and CMC Using the wider Healthcare Team to deliver ACP conversations Kate Kandasamy July 2021
Wandsworth created over 2000 CMC records over Wave 1 Some uncertainty about quality - particularly patient wishes, contact details and Treatment escalation plans PCN debriefs after Wave 1 highlighted issues around sustainability Expansion of Additional Roles in PCNs already happening and increasing in 2022 How could we encourage a broader approach to having ACP conversations?
Could using Paramedics be a solution?
Already independent Practitioners Already providing both acute and chronic home visiting services Probably already seeing patients we might not be identifying Experience of using CMC in an emergency so good insight into what is helpful More time than GPs
Successful recuited an experienced Paramedic Identified and engaged with a PCN to host the Paramedic Training and Supervision organised Rolled out Early Identification Toolkit across the PCN To date 30 new CMC records created Upskilling of other staff members within the PCN
Very positive feedback from Patients and Families Very positive feedback from GPs and Staff Increased awareness of ACP conversations within the PCN High engagement with my CMC - >90% It takes time - most records required 2 visits Supervision, training and connections all take time Logistics - IT set up, Parking! Resource allocation across Practices
Formal end of project assessment Quality, Patient and PCN feedback Roll out to other PCNs - initially planned for September Engagement with adjacent Borough Training plan to link with Paramedic Roadmap
