Background My mother, Dr. Consuelo Carmona, works at WSNM I have had people who are from her group ask me about their patients who are being cared for at UNMH after being referred through PALS Traditional methods of communication and transition do not meet patient or healthcare provider needs. Communication must be standardized to be understandable and be used by the IP team. Care must continue post- discharge by utilizing technology to increase quality and continuity of care.   3. Premise of Project Understand perception of community regarding referrals to UNMH Provider survey to explore how providers would prefer to receive information/what they currently receive from UNMH referrals

  4. Survey Questions What organization are you from? How much interaction do you have with UNM OB/GYN residents? How do you currently obtain information on your patients transferred to UNM? What information would you like to have when your patients are transferred to UNM? What is the best way to update you regarding your patients that are transferred to UNM? Would you be willing to share your contact information as part of a state-wide collaborative to improve communication between OB/GYN providers in the state?

Survey Responses 8 total responses, from WSNM(unable to contact Presbyterian Medical Group/Lovelace Medical Group) 12.5% reported receiving communication through providers they know on a personal basis 25% received information about their patients transferred to UNM through directly communicating with the patient 50% reported not receiving ANY information on patients that are transferred to UNM Also reported asking residents who rotated with them at Lovelace/Presbyterian 100% would want discharge summaries from their patients that were transferred to UNM 75% would want communication with their office, i.e. fax, to receive information about their patients

Future Direction Obtaining preferred contact information for referring provider groups to Keri Freeman/Care Management Team Improving PALS system for ensuring adequate follow up after referral Ensuring, with multiple system levels, that patients leave with discharge summary vs. have DC summary faxed to referring provider Extending provider survey to community providers that refer to UNMH outside of Albuquerque Developing statewide provider repository for enhancing communication

  7. Future Direction 1. Extend the provider survey to more community providers that refer to UNMH both within and outside of Albuquerque. 2. Obtain preferred contact information for referring provider groups and provide to Keri Freeman/Care Management Team and care teams. 3. Include the PALS system in ensuring follow-up. (e.g. add to a OB Transport Care team). 4. Build a system, with redundancies to ensure that patients leave with discharge summary and have DC summary faxed to referring provider. 5. Develop a statewide repository for provider contact information.
