  2. Exercise 1 Your supervisor has assigned you the responsibility for creating the 2018 inspection plan for the NPP. TASK Working in groups of three : Determine which safety areas or key activities will be assessed in 2018. For each inspection determine the inspection topic, the number of inspectors, number of days and the month of the inspection. Exercise 2 The annual inspection plan has been approved and you have been assigned as the lead inspector for the first inspection at the NPP TASK In the same groups of three, prepare the inspection, including: Daily schedule Inspection scope Major inspection activities List of persons the team will interview List of activities the team will want to observe List of documents the team will review List of checklists the team will need to prepare (do not prepare the actual checklists) Prepare the list of questions for the interview of the senior manager at site TIME 45 minutes to prepare 5 minutes to present to other groups Inspection and enforcement 25 September - 6 October 2017 3

  4. Exercise 3 The inspector found, during an inspection on a ongoing worksite, a highly corroded fire water pipe. This corrosion problem makes the fire water distribution network in the administrative buildings near the turbine hall unavailable. The inspection gave rise to a number of findings including: - inadequate maintenance on fire-fighting water systems; - failure to carry out a periodic test on the fire protection of the oil tanks of the turbo-generator unit of reactor 2, due to the poor condition of a pipeline. In conclusion, the inspectors noted a serious lack of preventive maintenance on the site's fire water systems. A programme to replace certain peripheral pipelines is planned for the end of the year, but it will be insufficient to deal with problems encountered on other pipelines.   5. Exercice 4 (1/3) You have completed the inspection and have identified the ten inspection findings listed below: 1. The inspector noted that several relief valves from one supplier had been received the previous week. The NPP QA group tested one of the relief valves and found it did not open until well above set pressure. The valve was reset, but no other similar valves were tested. 2. found that system had not been installed according to the flowsheet. The NPP supervisor for the system approved the change in the field, and stated the flowsheet will be updated within 30 days. On checking flowsheets for a system important to safety, the inspector 3. them were not signed. On reviewing completed work order forms, the inspector found that 10% of 4. inspector discovered that the NPP knowingly installed a counterfeit component. While reviewing a recent installation of the reactor shutdown logic, the 5. pump. Later on, the inspector checked the training records for the worker and could not find a qualification for torqueing bolts. The NPP supervisor confirmed that the worker is not qualified to torque bolts.   6. Exercice 4 (2/3) You have completed the inspection and have identified the ten inspection findings listed below: 6. While walking around the site, the inspector noticed a number of unrestrained gas bottles. 7. that were blocked by construction material. While walking around the site, the inspector noticed two fire extinguishers 8. inspection. Upon checking the training documentation, no record of qualification could be found for one of the ten welders observed. The welding supervisor stated that this welder was the best welder he had and that the person was highly qualified. The inspectors observed a number of welding activities during the 9. last time. The inspectors reviewed the documentation and could not find a record indicating that the system had been checked for foreign material before being sealed up. The week before the inspection, a major system had been sealed up for the 10. At the end of the day, the inspector does a tour and finds an unfinished construction activity. There are personnel safety hazards, sensitive equipment waiting to be installed, and tools laying around. Inspection and enforcement 25 September - 6 October 2017 6

  7. Exercice 4 (3/3) TASK In different groups of three, discuss each finding and: 1. Rank the findings in order of importance (most serious first). 2. Determine what action you would place on the NPP to get the NPP back into compliance and prevent recurrence. Also discuss the timeframe for the NPP to return to compliance 3. Discuss the immediate measures you would expect the licensee to take. 4. At this time, do not consider enforcement sanctions or penalties.
