Homeless Initiative B7 Homeless Initiative B7- -Interim/Bridge Housing Interim/Bridge Housing For Those Exiting Institutions For Those Exiting Institutions RECOMMENDATION Direct the Los Angeles Homeless Services Authority, in collaboration with the Department of Health Services (DHS), Department of Mental Health (DMH), Probation Department, Department of Children and Family Services (DCFS), and the Sherriff (LASD) to develop and implement a plan to increase the interim/bridge housing stock across the County, including identification of funding that can be used to support the increase. 2

  3. RECOMMENDATION Direct the Los Angeles Homeless Services Authority to enhance the emergency shelter system, as described within. DESCRIPTION Homeless Homeless Initiative E8 Initiative E8- - Enhance the Enhance the Emergency Emergency Shelter System Shelter System The emergency shelter system should be enhanced to be an effective point of access to and component of an integrated homeless services system. An adequate crisis housing system ensures that individuals, families, and youth have a safe place to stay in the short-term, with access to resources and services that will help them exit homelessness quickly-optimally within 30 days. EMERGENCY INTERIM/BRIDGE HOUSING TYPES DHS/DMH Interim Housing-serving individuals with complex health and/or behavioral health conditions who need a higher level of supportive services. Stabilization Housing provides case management and other supportive services Recuperative Care provides case management, other supportive services and health oversight 3

Who is eligible for Interim Housing Individuals exiting institutions include jails, prisons, foster care, hospitals, urgent care centers and other medical, behavioral health, and substance abuse treatment facilities. Homeless Homeless Initiative B7 Initiative B7- - Interim/Bridge Interim/Bridge Housing Housing individuals with complex health, mental and/or behavioral health conditions who need a higher level of support services than is available in most shelter settings. Individuals currently homeless on the streets. Individuals eighteen years of age or older and families with minor children 4

There is no length of stay. DHS Stabilization/Recuperative Care DHS has contracts to provide interim housing services for single adults, women and children, including pregnant women with a total of 26 sites. There is no length of stay. Current Portfolio: 72 families 397 individual Current Portfolio: 758 Stabilization Bed 494 Recuperative Care Beds 5

Interim Housing Programs Interim Housing Programs DHS DMH Housing For Health Interim Housing Program (IHP) Stabilization Housing Recuperative Care IHP Provides short-term care and medical oversight to homeless individuals who are recovering from an acute illness or injury or have conditions that would be exacerbated by living on the street or in shelters. Provides bridge housing and support for homeless individuals prior to permanent housing placement. Provides temporary shelter services to adults with mental illness and their families who are committed to transitioning to permanent housing. 6

DHS DMH Housing For Health Interim Housing Program (IHP) Interim Housing Program (IHP) Stabilization Housing Recuperative Care Receiving mental health or outreach and engagement services Medically and psychiatrically stable individuals who may be vulnerable to decompensation if not placed into shelter housing. HIV/AIDS Diabetes Hepatitis Mental Health (e.g. Schizophrenia, Depression, Bipolar, etc.) ETOH Medically and psychiatrically stable patients requiring low- level medical oversight for: Wound care Recovery from surgical procedure Need additional time to recuperate from illness and/or injury Common Health Conditions: End Stage Renal Disease Congestive Heart Failure (EF > 20%) Cancer Decubitus (Stages I-III) Serious Mental Illness Not have adequate benefits or other financial resources to pay for temporary housing Not be on conservatorship or require care and supervision 7

  8. Program offers temporary shelter in a stable environment to assist clients in increasing independence and completing housing goals. Room and Board Interim Interim Housing Housing Services Services 3 meals per day Allow clients to stay in facility 24/7 days a week Hygiene supplies Laundry services Secured/personal storage for medication Ensures a whatever it takes & harm reduction approach 8

  9. - Mental/Behavioral Services - Full biopsychosocial assessment - Mental Health treatment and substance abuse counseling - Regular ongoing evaluations and monitoring of mental health status Interim Interim Housing Housing Services Services Cont. Cont. - Case Management Services - Assessments of clients needs - Assistant with obtaining Identification, birth certificate - Assistance and advocacy with benefits -Linkages to supportive services (mental health, substance use treatment) - Linkages to transportation - Assistance with life skills and community participation 9

LAHSA or Health Agency B7 housing coordinator reviews each referral and works collaboratively with the referrer to identify a placement in an appropriate level of care and in consideration of geographic needs/sensitivities. LAHSA/Health Agency B7 housing coordinators receive referrals and review to determine which B7 environment (emergency, bridge, stabilization or recuperative care) is best fit. Interim/Bridge Interim/Bridge Housing Housing Referral Process Referral Process For LAHSA Bridge or Crisis beds individual has few primary issues that are not severe CES score of 1, 2 or 3 with an acuity score of 7 or below. Applicable housing coordinator communicates and coordinates transfer with the referrer. For DHS/DMH Interim Housing (Stabilization, Recuperative Care, and Interim Housing Program individual has several primary issues that are complex and challenging CES score of 3 with an acuity score of 8 or above. For DPH-SAPC Recovery Bridge Housing, Health Agency housing coordinator submits the referral to SAPC Utilization Management Unit staff. 10
