  1. What is SIDeR+? SIDeR+ is a clinically led digital programme that aims to make the right information available to the right professional at the right time Black Pear Software is our current Technology Partner since 2018 The first SIDeR+ service, the Electronic Palliative and Care Coordination System (EPaCCS) went live in April 2019 The SIDeR+ Shared Care Record (SSCR) went live in November 2020 The SIDeR+ Programme is much broader than just the Shared Care Record, it is now used as a skeleton key to overcome limitations with digital systems Includes a suite of read / write / create Shared Forms designed to be a single version of the truth 2

Succinct real time dataset that is not persisted For direct care purposes only For use by health & care professionals Clinically led, digitally enabled content, shaped by input from staff and the public Flexible, to morph and change according to need Contains an active audit log Promoted internally, with stakeholders and the public using forums, social media and traditional media 3

Somerset NHS Foundation Trust Somerset Council (Adult Social Care, Children s Social Care and Public Health Nursing) St Margaret s Hospice 62 Somerset GP Practices 5 Dorset GP Practices The Blackdown Practice, Devon + Digital Shared Forms one version of the truth - Somerset Treatment Escalation Plan (eSTEP) - Special Patient Notes - Advanced Care Plan - Open Mental Health Service (x2 forms) - Comprehensive Assessment Form - Personalised Care and Support Plan and About Me

SIDeR+ continues to improve access to information to benefit patient care SIDeR+ is being accessed 37,000 times per month by nearly 3,000 different staff members across Somerset to access over 16,000 individual patient records Over 46,000 Care Plans are hosted on SIDeR+ - one version of the truth Contextual launch from native clinical systems saves time logging in Patients spend less time repeating their information

  5. Recent Projects SIDeR+ has achieved several enhancements over the last 6 months. These improvements are: Access to documents held in the GP system and by St Margaret s the launch of e-STEP Access past medications held in the GP system access to Dorset surgeries feeding into YDH Sharing copies of care plans to the National Care Record Service Medicines reconciliation via Better Platform led by Somerset FT Our next venture is to improve the User Interface of SIDeR+ including data from Out of Hours and SWASFT
