Vision Statements: Dignity & Respect Vision Statements: Dignity & Respect ODC - Person-centered conversations with people to know what is important to them. Opportunity - All the people we support understand what it means to be treated with dignity and respect. Headwaters - Individual choice, person-centered, acceptance/valued, belonging
Vision Statements: Dignity & Respect Vision Statements: Dignity & Respect DSAW will respect and treat all people as unique individuals through person-centered policies, procedures, and practices Aptiv - All individuals have potential to be realized and listened to LOV - Dignity and respect are present in all interactions, experiences, and relationships for members of the LOV community.
Vision Statements: Relationships Vision Statements: Relationships ODC - Build trust and maintain connections in community without staff support. Honor dignity of risk. Opportunity Support people who want to have significant others find those healthy relationships. Headwaters - Education, access, positive, healthy, safe, problem solving, support resources, opportunities, and experiences

Vision Statements: Rights Vision Statements: Rights ODC - Educate all staff on rights and why rights promotion is important. Provide rights info to participants and families (how to exercise). Promote self-advocacy and have resources on hand and readily available to provide. Opportunity - People know what their rights are and understand how to advocate for themselves. Know who to talk to for help and rights. Headwaters - Knowledge. All all have rights resources, support, education, person- centered, and advocacy education.
Vision Statements: Rights Vision Statements: Rights DSAW will respect, promote, and uphold the rights of all. Aptiv - Everyone understands their rights and can promote and exercise those rights. LOV Members know and understand them, feel supported in advocating and respect others' rights.
Goals/Action Steps: Dignity & Respect Goals/Action Steps: Dignity & Respect DSAW will increase the number of self-advocates on boards and committees Aptiv will incorporate person-centered planning tools, including the Positive Personal Profile, and create a safe and comfortable environment for people to share their dreams and goals ODC will create opportunities staff and participants to give feedback/express if their day is or is not going the way they want CRC will develop a Human Rights Committee LOV will provide education around dignity and respect for members, families and staff.
Goals/Action Steps: Relationships Goals/Action Steps: Relationships DSAW will create a mentorship program for staff. They will expand their person- centered planning pilot. Opportunity will increase the number of participating members in their Building Full Lives services.
Goals/Action Steps: Rights Goals/Action Steps: Rights Opportunity will increase the number of participants in their self-advocacy group in Wausau. They will start a self-advocacy group in WI Rapids and use the Rights Guides with these groups. ODC case managers will identify any current rights restrictions that may be occurring with people. They will first provide education to case managers on guardianship and rights restrictions. Will create a spreadsheet to document rights restrictions (get baseline) and develop plans to address. LOV will use the Rights Guides with bridge builder members and promote a workshop series to more members and use surveys to determine what was learned. Headwaters will educate people to know and exercise rights and responsibilities by forming groups with all interested participants to complete the Rights Guides together. Aptiv will start a rights workshop with Camille. They will implement an assessment to determine what people know about their rights
Pilot sites will further develop and implement plans with support from CQL Further Next Next Steps Steps Project team will capture lessons learned and incorporate into the Living Well HCBS Toolkit Capture
