  1. EPAS MDS-HC Overview Allison Godwin, RN

  2. The MDS-HC is an assessment tool that must be completed in person*. The assessment is to evaluate the client s ability to perform skills. It is not an assessment to evaluate the ability of the caretaker(s). Scores are to reflect how well the client can care for themselves. It s important to utilize the key when completing this assessment. Be sure to use the indicators that the key gives. Remember, check marks are not acceptable. Complete the MDS-HC as thoroughly as possible. *unless approved by EPAS Program Specialist Version 2.0 July 21, 1999

  3. Informal Support Services It s important to give accurate information here in order to reflect all needs. This is related to Section P: Formal Services.

  4. Instrumental Activities of Daily Living Are not considered necessary for fundamental functioning, but they let an individual live independently in a community. A-Be sure that this section reflects the client s self performance during the last 7 days B-With what degree of difficulty did the client perform this activity themselves? The reference to the timeframe is important in order to accurately reflect the client s ability to perform the function.

  5. Activities of Daily Living Routine activities that people tend to do everyday without needing assistance. Eating, bathing, dressing, toileting, transferring, and continence. Reference to time period is important. These scores shouldn t reflect occasional need. Please utilize the key when completing this section of the MDS-HC. These findings help justify the services scheduled on the service plan

  6. Its important to list diagnosis here. You shouldn t be guessing nor listing symptoms. ICD-10 codes are not required at this time. Remember to utilize the key. No check marks.

  7. This section should reflect all services that the client is currently receiving. This section should reflect all medications that the client is currently receiving. Including the route/dosage/frequency. This information should be legible and easy to understand. It is acceptable to attach a list if the list of medication is length.
