
Exploratory Project on Preventable Heart Disease Readmissions
This project aims to identify factors contributing to preventable readmissions of heart disease patients in low-to-middle-income families. With the goal of reducing hospital readmissions, the project focuses on engaging nurses, caregivers, patients above 65 years, and community stakeholders. The methodology includes interviews, thematic evaluation, implementing interventions, and budget planning for a comprehensive approach. The interdisciplinary team comprises professionals from nursing, communication studies, respiratory therapy, social work, communication disorders, and research, collectively known as TEAM HEART.
