  1. 4th Grade: Foothills Elementary Mustang Pride Award Mustang Pride is an optional extracurricular award program, that is completed primarily at home. If a student desires to receive a Mustang Pride Award, they must complete 14-20 of the following requirements. Once a requirement is completed, it needs to be signed in the appropriate box below. (T = Teacher signature required, P = Parent signature required.) Keep this sheet and any additional forms in a safe place and turn into your teacher by May 19th for A & B tracks and June 9th for C & D tracks.

  2. Complete 18-20 requirements for the GOLD Medal Complete 16-17 requirements for the SILVER Medal Complete 14-15 requirements for the BRONZE Medal

  3. *If a presentation or test is required, the student must schedule with the teacher in advance.

  5. BE PROACTIVE #1 BE PROACTIVE T 1. Attendance: No more than 5 absences or tardies throughout the year. T 2. Homework: Attend all Reward activities. T 3. Pass off multiplication Rocket Math (talk to your teacher for requirements) T 4. Pass off all KBK typing lessons 1 - 23.

  6. BEGIN WITH THE END IN MIND #2 BEGIN WITH THE END IN MIND T 5. Independent Reading read at least 400 minutes a month for 8 months T 6. 80% or higher on 4 math tests T 7. 80% or higher on 4 science tests T 8. 90% or higher on 20 spelling tests T 9. Go up 100 points on the SRI test or go up three guided reading levels

  7. PUT FIRST THINGS FIRST #3 PUT FIRST THINGS FIRST P 10. Plan and carry out a family activity, outing, project, lesson, etc. Write a reflection on how it went. T 11. Memorize and label Utah s 29 counties on a map. 12. Memorize and label Utah s mountains, rivers, and lakes on a map. T

  8. THINK WIN-WIN #4 THINK WIN-WIN P 13. Perform 8 or more hours of student-chosen service, done outside the home. Does not include chores or paid services. T 14. Complete and submit a 7 Habits project/presentation (ideas could include poster, skit, song, book, poem, PowerPoint, movie, etc.)

  9. SEEK FIRST TO UNDERSTAND TO BE UNDERSTOOD #5 SEEK FIRST TO UNDERSTAND THEN TO BE UNDERSTOOD T 15. Complete a report on something Utah related (environment, mountain man, pioneer, famous Utahan, current event, etc.). Prepare a 3-5 minute oral presentation with a visual-aid (poster, PowerPoint, diorama, written book, etc.) T 16. Interview a parent, grandparent, or older neighbor about what their life was like as a child. Write a three paragraph essay on the difference between their life and yours.

  10. SYNERGIZE #6 SYNERGIZE T 17. Participate in two school-wide events (Reflections, Science Fair, Crossfit Fun Run, Literacy Night, etc.) P 18. Participate in one performance in front of an audience during the year. (Recitals, class or school talent shows, grade program, religious program, etc.)

  11. SHARPEN THE SAW T 19. Memorize and pass off My Country Tisof Thee on the recorder. P 20. Complete Passport to Fitness over the course of the school year.

  12. Due Dates A & B Tracks due May 19th. C & D Tracks due June 9th.
