PROGRAM OVERVIEW Purpose: Provide reassurance to isolated individuals that we are committed to ensuring their well-being Facilitate a simple way for connecting to area agencies on aging, if desired
The Ohio Department of Aging (ODA) is offering a FREE, daily check-in by phone for Ohioans age 60 or older. This service can also connect you with aging network information and support. Register online at www.aging.ohio.gov (preferred, if able) -or- Call 1-833-ODA-CHAT / 1-833-632-2428 3
PROGRAM OVERVIEW Check-in calls will be made from 7 a.m. 6 p.m. daily. Participants choose a time that suits them. Participants can place a hold on these calls at any time. 4
WHO CAN PARTICIPATE Ohio residents age 60 or older who: Have a valid phone number (mobile or landline). Provide an alternate contact (preferred but not required). 5
HOW IT WORKS ODA will make three attempts to contact the participant. When participants respond, they have these options: If no answer after three attempts, ODA: Reaches out to the alternate contact, if one is provided. Confirm they are OK. Places a live call to the participant and alternate, if previous contacts fail. Transfer to learn more about aging services in your area. Notifies local non-emergency services (e.g., sheriff s office) if live call fails to connect.* *This is a non-emergency service. If you have an emergency call 9-1-1. 6
REGISTRATION INFORMATION Information to register: Ways to register: Name, year of birth, street address, phone number, and email (if available) Online at www.aging.ohio.gov (preferred, if able) By phone at: 1-833-ODA-CHAT 1-833-632-2428 Name of alternate contact, relationship to the participant, their phone number Who can register: The preferred one-hour contact window between the hours of 7 a.m. 5 p.m. Participant on their own behalf 7
