  1. Funding Opportunity for Local Public Health Agencies December 7, 2021

Initiative Objectives DHSS seeks to partner with Local Public Health Agencies (LPHAs) from across Missouri to implement strategies that advance health equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved. Proposed activities will align with at least one of three goals: 1. Reduced COVID-19 related health disparities, 2. Improved and increased testing and contact tracing, or 3. Control COVID-19 infection/transmission

Funds Available $3M to $12 M is available across all funded LPHA projects LPHAs will be contracted for approximately 12-16 months beginning in early 2022 Award minimum: $100K Must submit simple budget for proposed activities No ceiling on award budget; approved amount depends on several factors, including: funding needs for proposed activities subcontractors and partnerships total number of LPHAs participating across the state

Application Process and Timeline DHSS has provided a Project Overview Template Sections roughly map to typical contract components Project Activities, Deliverables and Outcomes, and Budget are key components Basic information and Project Overview Template must be completed and uploaded through formsite page: https://fs2.formsite.com/openform/qjd6flgckd/index.html There will be two rounds of funding: First deadline for applying is 5pm, Friday, Dec. 17th Second deadline is 5pm, Tuesday, Jan. 11th After your Project Overview is received, a member of SHDI will contact you if there are outstanding questions as they consider your contract needs After round one allocation decisions are made approximately January 1, 2022 DHSS will provide updated available funds for the second round
