  1. Accommodation Notification Letters & Requests for Additional Accommodations

  2. Steps to Generate your Accommodation Notification Letters 1. If it is your first time logging into myAccess for the current semester, you will be required to read and electronically sign any required forms that the Disability Center has pertaining to specific accommodations. 2. You will see a list of the classes you are enrolled in. Click the checkbox next to those classes in which you plan to utilize some or all of your accommodations. Click Step 2 below the yellow box titled Rights and Responsibilities .

  3. Steps to Generate your Accommodation Notification Letters (cont.) 1. Choose the accommodations you wish to utilize for each specific class by clicking the checkboxes next to each group. 2. Click Submit Your Accommodation Requests

  4. Steps to Generate your Accommodation Notification Letters (cont.) Your accommodation notification letters will be emailed to your instructors within 2- business days by the Disability Center. Some accommodations require approval by your access advisor each semester and will list as Pending until approved. *Remember, once you have requested your accommodations, arrange a time to meet with each instructor and discuss the specific details of implementing your accommodations.

  5. Accommodation Plan Modification Request Form If you would like to request additional accommodations other than the ones you have already established with your Access Advisor you will need to complete the Accommodation Plan Modification Request Form . The request form is located in the Home section of your myAccess account. After you ve submitted the form, please allow 3 business days for your Access Advisor to contact you about your request once they have reviewed it.

  6. Accommodation Plan Modification Request Form (cont.) 1. Click on Submit Additional Accommodation Request Form. 2. Fill out the form based on the questions. 3. Click Submit Application when finished. Once you submit the form, the office and your Access Advisor will receive a copy and begin to review. You can expect contact within 3 business days.
