Disclosure I am a Certified Vestibular and Concussion Specialist Athletico Physical Therapy: I treat at the Niles/Northwest Chicago on Touhy Ave IL Regional Concussion Coordinator Manage Baseline Testing Services for Athletico Concussion Consultant: consult and act as liaison at Collegiate level, specializing in RTP (return to play) Teach Concussion and RTP courses across the organization on proper management
Objectives Recognize signs and symptoms of a concussion Understand recommended next steps after concussion Have awareness on what role you play in the athlete s recovery process Learn when to seek additional advice Gain awareness on resources available
What is a concussion? Brain as an egg mTBI Complex pathophysiological process Trauma induced Change in mental status Symptoms may be subtle Some symptoms may not surface until 24-48 hours after Recovery from a concussion is different for everyone
Myths You have to have loss of consciousness Only a result of a direct blow to the head You need to check pupils with a flashlight A concussion will show up on an MRI 3 concussions and you re retired
Signs and Symptoms Symptoms: Headache Dizziness Blurred Vision Nausea Light/Sound sensitivity Foggy Difficulty concentrating or remembering Fatigue Confusion Drowsiness Signs: LOC Poor Balance Slurred Speech Confusion Vomiting Not acting like themselves
What do I do as a parent? Assess: Symptoms, neck movements, coordination, orientation x 3, memory, concentration ED is not the answer Red flags to warrant ED visit: Uncontrolled vomiting, paresthesia, worsening headaches, slurred speech that progresses, LOC throughout the day Old wives tales Proper sleep, nutrition, hydration Seek an evaluation from a CONCUSSION trained professional
What do I do as a coach? Not every impact that causes symptoms is a concussion Not your job to diagnose Assess: Symptoms, neck movements, coordination, orientation x 3, memory, concentration Other things that mimic concussion: dehydration, exertion in heat, migraines, trauma, and cervical strain When it doubt sit them out Overall awareness and communication Accountability to RTP Lean on your organization and hockey directors
Proper Management Relative rest with gradual exposure to normal life Strenuous activity should be avoided or limited Avoid excessive over the counter meds unless directed by a physician Contact school nurse, guidance counselor, administrator for accommodations
Lingering Symptoms Not getting better with gradual exposure to normal activity Therapy is usually warranted Neck Oculomotor Vestibular Exertional Behavioral Cognitive RTP assistance
What is baseline testing and what does it mean? ImPACT, $15 offered at some Athletico clinics across the region, ends Nov 1 Does NOT diagnose concussions Used as a tool for medical staff when deciding RTP Your team/coach does not need results, just receipt
When can my athlete return to play? Cleared by a physician, with a written note to return Graduated RTP by a licensed healthcare professional Athlete may feel fine at rest, but symptoms may be exacerbated with exertion
Who can clear my athlete? Under state laws, an MD, NP are the ONLY medical professionals allowed to clear for RTP after diagnosed concussion Athletic Trainer or PT able to guide through RTP protocol
