Why should we align these efforts? Significant gaps in services across Nevada, especially Rural counties Only Transportation and Aging and Disability Resource Centers are serving consumers in all 17 counties. State Plan for Aging Services How we do our work Purpose: It includes all plans to be conducted by [DHHS, ADSD] under the provisions of the Older American Act. Focus Areas: Older Americans Act Core Programs ACL Discretionary Grants Participant-Directed/Person-Centered Planning Elder Justice Quality Management Data collection to assess ongoing program implementation Remediation of problem areas Continuous improvement Helping People.
