Supported Living - Overview AWD (Adults with Disabilities) Mental Health Contract MHSL has 2 Lots Mental Health Support Mental Health Support with Personal Care CQC registered provider. 23 Providers on the list AWD (Adults with Disabilities) Learning Disability, Autism/Neurodiversity, Physical Disability and Sensory Impairment Contract 4 Lots 45 providers on these lots Good choice of local, regional and national providers Info on the lots and provider profiles can be found on SharePoint. Open contract with yearly joining on periods

  4. Adults 18+ What is Supported Living (and why is it different to Home Support?) SupportedLiving (SL) can take place in supported accommodation or in a person s own home Supported living can also cover community support which isn t covered by home locality contracts. Personal care packages regular daily calls are not SL they will go through the home support contracts. Any care and support would need to be assessed by a Social Worker Often there can be elements of core support as well as personal support.

Support Planning with the Adult identifies that these needs can be met by having a package of care which consists of To reside in Supported Accommodation with some Shared Support (core hours), *LD Only - Any Personal Hours (which relate specifically to support at the supported accommodation) and Any Community Support (which is directly linked to outcomes outside of the supported accommodation support) or any other personal support if MH Support Plan identifies outcomes. Adult has eligible, unmet need. Support Planning with the Adult identifies that these needs can be met by having a package of care which consists of: 1 hour support 3 x per week to attend activities and groups in the community OR Support Plan identifies outcomes. Manager approval referral to QAF Service line added Supported Living Community Based. Approved by manager Manager approval referral to QAF Approved You ll see a shift here for both AWD and MH differentiate between what is personal hours linked to accommodation needs and community support linked to other outcomes. No personal hours in MH, all 1:1 support is under community based. Sent to Support Options. NOTHING CHANGES HERE

Process for Finding a Provider: Information is available on Sharepoint: Mental Health: Adult Social Care Information Portal - Mental Health Adults with Disabilities: Adult Social Care Information Portal - Supported Living Connect to Support Directory Search | Bradford Connect to Support It is expected that the Social Worker will have considered the locality of providers, any specific knowledge of skills required to support the individual and whether personal care is required (asnot all Supported Living Providers may be registered to provide personal care/CQC).

Service Lines Add the Service Line Important information needed in the notes section Workflow to Manager who checks and approved Workflowed to Support Options by manager Support Options add the service line Important to note!! This must be done as soon as possible, preferably before the service starts there must not be a delay.

  8. Guidance Service lines AWD The assessor is responsible for making the primary determination of which type of support the individual is to receive. If you have any queries regarding the shared hours in any accommodation, then the assessor must discuss these with Contracts colleagues and the Homefinder team. All 1-1 support can be two categories: o Supported Living: Personal hours (LD only) attached to accommodation. This is where the personal hours are for personal cares/personal support within the community-based home. These hours are not community based. o Supported Living: Community-based Support. These hours are attached to an individual and primarily used for social inclusion type activities. Following Care Act Review, where an individual currently has 'Personal Hours' , the review should indicate where any of these hours may need to be moved under 'Community Support' and this should be actioned. Please note the assessor will also have to clarify and add appropriate service lines for a property which could include the following: Supported Living - Waking Nights Supported Living Sleeping Nights Supported Living Core Hours

Service Line Template AWD(LD) The following is an example of service notes and information Support Options colleagues will require to ensure all services are set up and processed correctly (there should be a service line for every type of service, core, waking and sleeping). Service Notes PSR: Learning Disability/Physical Disability Support etc. Budget: stating locality team responsible. Provider: Provider Name & Property address if shared support (Core Hours/Waking Nights/Sleeping Nights) OR Provider Name AWD Community Based Single/Double (Delete as appropriate) Start date: xx/xx/xx This should match the start date in the support plan, and the start date confirmed with the provider Weekly hours: xxxx - where this is shared support, this should be noted eg xxhrs is individual share of core support Pricing Agreement: Supported Living rate xxxx (this should match what is in the support plan, anything outside of a standard rate will not be processed without additional confirmation of authorisation) Details of support: Notes indicating outcomes stating whether personal hours or community-based support. IF split package additional information is required E.g.: Total weekly hours -xx 50/50 funded with ICB - CHC or S117 needs to be clear LA = xxxx @ xxx = xxx ICB = xxxx @ xxx = xxx

  10. Guidance Service lines AWD (MH) If you have any queries regarding the shared hours in any MH schemes, then the assessor must discuss these with Contracts colleagues. All 1-1 support should be put on as service type Supported Living - Community Based Single or Double Handed and not Supported Living Personal Hours as previously.
