PUBLIC COMMENT PERIOD The comment period begins on November 1, 2016 and ends on December 19, 2016. This timeframe allows an additional period of 15 days for the public to comment after the last public meeting. DDDS Lifespan Waiver Amendment
Background: DDDS directed by Budget Act to plan for a Family Support Waiver
Approach: DDDS will amend the current Medicaid HCBS DDDS waiver, to create the Lifespan Waiver The Lifespan Waiver will: Add new services targeted to families Add waiver slots to include people with IDD living at home with their family
This Makes Sense for Delaware: Enable smooth transitions as needs change Increase Administrative Efficiencies Performance measure tracking and reporting Provider Enrollment
How Much Will This Cost? Expected to be budget neutral for the service costs Uses current DDDS funding of $1.2M for family support services to leverage federal Medicaid funds Preserves state funds for individuals who are not enrolled the waiver and for costs the waiver does not cover
Who Will Be Served? Individuals already enrolled in the DDDS waiver Individuals, age 12 and above who currently receive DDDS Day Services plus new school graduates each year (about 1,100 initially) New individuals in need of residential services because they cannot be supported in the family home
Services Included In The Lifespan Waiver: All DDDS waiver services currently provided PLUS
Services Included In The Lifespan Waiver (cont d): New Family Support Services: Community living support (respite & personal care) Community Participation
Services Included In The Lifespan Waiver (cont d): New Family Support Services: Home or vehicle modifications Assistive technology Specialized medical equipment and supplies
What Is Different About The Lifespan Waiver Will Be Managed? Person Centered Planning Case Management
Person-Centered Planning: Lifespan Plan for individuals in a residential setting AND Support Plan for Individuals and Families for individuals (SPIF) living in the family home Both planning processes are designed to be fully compliant with the CMS HCBS Rule
Case Management: Targeted Case Management (TCM) will be available for: Individuals living in a DDDS residential setting Individuals living with their family
Will The Staff Who Support Me Change? For individuals living in a residential setting -- DDDS case managers will retain responsibility for case management For individuals living with their family -- The private TCM provider will hire Community Navigators to provide case management.
Transition Process: In the Spring of 2017, DDDS will begin: A streamlined, phased in enrollment process Referrals to the Targeted Case Management Agency Information gathering to start developing the person centered plan
Post Transition: After the initial phased-in mass enrollment As new individuals leave school, they will be assisted to apply for the waiver by their Community Navigator
