Documents Needed(required) 3 Referral Referral is required from CES for Housing Programs. HOPWA Program- Requires Referral and Diagnosis Housing Programs Requires and Assist Vital documents including ID, Social Security Card, Birth Certificate. Support Services 4 Support Services We assist with credit check/application fee, PG&E security deposit, acquisition of vital documents. Access Point- Send and receive referrals and complete assessments to best help the client. (VI-SPDAT, Diversion, HACK referrals) Transportation Assistance (Bus Pass)(Company vehicle) Financial Assistance- Connection to recourses Social Security, Department of Rehabilitation, DHS, etc. Housing Needs- Appliances, Food, Clothing, Medical Clinica Sierra Vista

Kern Health Systems Project 6 o Permanent Supportive Housing Program o CESH Proposal serve 25 household House Homeless individuals or families who are insured under Kern family Health Systems and reside within Kern County. This persons are matched from CES. Provide intensive Case Management services to ensure and maintain stable housing Clinica Sierra Vista
Housing Opportunities for persons living with AIDS (HOPWA) 7 o Offers one-on-one case management services to low-income HIV/AIDS clients to coordinate housing, healthcare, and social services needs for improved overall health. o At Risk - Short-term stabilizing support with rent; mortgage and utility assistance is given to clients to help reduce the risk of homelessness. o Limited Hotel/motel for HIV/AIDS Homeless individual, permanent housing placement services, meals, nutrition, and transportation Clinica Sierra Vista
