  2. Definition Formulary is a continually revised compilation of pharmaceuticals and some important ancillary information that reflects the current clinical judgement of medical staff. Aim of Hospital formulary : To provide best possible treatment to the patient in lowest possible cost

  3. Guidelines for preparing Hospital formulary Members of PTC should participate in deciding the drugs to be included in formulary. Drugs are selected depending upon the specific needs of hospital. Drugs should be included in formulary by using their non-proprietary names. As per the needs of particular hospital its formulary system should be adopted by medical staff Copies of formulary must be readily available at all times.

  4. Contents of Hospital formulary 1. Pharmacological index 2. Alphabetical index of drugs 3. List of emergency drugs 4. List of narcotics and dangerous drugs 5. List of drugs with their uses, doses and dosage forms, route precautions, contraindications and adverse drug reactions of administration,
