Responsibility of hospital pharmacist INPATIENT PHARMACIST RESPONSIBILITY: CENTRAL DISPENSING AREA. PATIENT CARE AREAS. DIRECT PATIENT CARE AREAS. GENERAL RESPONSIBILITY.
SET UP OF HOSPITAL PHARMACY:- PATIENTS CLINICAL STAFF HOSPITAL ADMINISTRATION DEPARTMENT OF PHARMACY NURSING STAFF HOSPITAL COMMITTEES OTHER DEPARTMENTS The Hospital pharmacy department have the interaction and provides services to the above mentioned other all departments of a hospital.
CENTRAL DISPENSING AREA To ensure that all drugs are stored dispensed correctly. To check the accuracy of the dosage form prepared especially of: *Intravenous admixture. *Unit doses. To keep proper records & preparation of bill. To coordinate the over all pharmaceutical needs of the patient care . To ensure that the established policies & procedures laid down are followed.
To maintain professional competence. To communicate with all pharmacy staff regarding new development in areas. To ensure that new personnel in his section properly trained in the policies & procedure of the central dispensing area. To coordinate the activities of area with available staff to make best possible use of personnel & resources.
PATIENT CARE AREAS To maintain a liaison with nurses who r involved in the drug administration to the patient in their charge. Reviewing of drug administration in each patient periodically to ensure that all doses r being administered . To provide instruction & assistance to the junior pharmacist as needed in dealing with patient & new procedure. To coordinate over all pharmaceutical services on the running unit level.
DIRECT PATIENT CARE AREAS Identification of drug brought into hospital by patient. Obtaining patient medication history & communicating al the information to attending physicians. To assist in the selection of the drugs product & their identities. To monitor patient total drug therapy for: *Effectiveness. *Side effect . *Toxicities. *ADR. *Drug interaction.
Counseling patient on: *Medication which r to b self administered in the hospital. *Regarding the use of discharge medication. Participating in cardio pulmonary emergencies by: *preparation of drug needed *charting all medication given
GENERAL RESPONSIBILITY To provide in service training & education for: *The Pharmacist. *Pharmacy student. *Nurses & nursing student. *Physicians & medical student *To provide information regarding drugs to physicians, nurses &other health care professionals.
OUTDOOR PHARMACIST RESPONSIBILITIES: CENTRAL DISPENSING AREA. PATIENT CARE AREAS GENERAL RESPONSIBILITIES.
CENTRAL DISPENSING AREA Proper technique are used in extemporaneous compounding. Provide for adequate record keeping & billing in respect of patient medication, record of investigational drugs, record of outpatient bills for charge of services & materials. Correctly maintaining prescription files. Keeps the outdoor pharmacy in a neat & tidy manner.
PATIENT CARE AREA To periodically inspect the medication areas on the nursing unit & to make sure that they are always kept supplied with drugs & other articles required. To identify drugs brought in to the clinic by the patient and record the patient medication history and communicating the same to physicians concerned. Monitoring of drugs as explained earlier. To counsel the patient on the proper use of medication & prepare the medication for intravenous administration.
GENERAL RESPONSIBILITIES To coordinate over all pharmaceutical needs, of the outdoor services area & exercise adequate control over such drugs. To ensure that all drugs are handled properly such as storage of investigational drugs. To participate in cardio pulmonary emergencies in the manner described. To provide for in services education and training for pharmacist, pharmacy items (the pharmacy student sent for practical training as a part of diploma or degree courses & nurses.)
