  3. Problem Identification Several cases reviewed at the regional trauma and MCA peer review BACKGROUND Findings/Gaps Identified and Hospital to EMS handoff is not standard practice GAPs No regional standard for handoff Lack of handoff resulted in harm or potential for harm

Regional PI project established in 2022 Stakeholders identified: LI IV trauma centers EMS Medical control/physicians RTN Workgroup established Multidisciplinary team created: TPM, TMD s, EMS medical director, EMS staff, Hospital mission control (patient movement and bed control), Regional TPC, Physician group quality coordinator, ED manager

  5. Improve patient safety during EMS transfer of trauma patients to a higher level of care by Workgroup s Purpose Statement provision of clear medical direction as communicated by effective handoff communication between the hospital and EMS using written and verbal reports.

Provides a CONSISTENT format Provides clear medical direction for the duration of the transport Physician orders The Interfacility Transfer Form (written report) Serves as a template for hospital staff verbal communication to EMS Several revisions with a multidisciplinary team before and after trials Need to Knows on one page Located on the WMRMCC website trauma page

ED to higher level of care transfers 2 Trials Level III and IV hospitals Audits & Feedback Trial 1: Feedback from audits and interviews showed staff resistance, redundancies in documentation, lack of buyin Revisited education (workgroup created an education tool), form revisions Changes & Education Trial 2: Better compliance, positive feedback from staff, physicians and EMS, good (not perfect) form completeness. RTAC/RTN voted support regional implementation RTAC education

Start date: July 1, 2023 Rollout education at June RTAC education session Regional Implementation Regional RTC (Helen) will follow up with each hospital and EMS as needed Will provide each hospital with: Education Tool a one pager of information on the project Suggestions for education and follow-up method Resources IFT and Education Committee, RTC

  12. Audit 2 separate weeks within the 6-month window to coincide with the RTN Inventory: Jan- June and July-Dec. Auditing # of transfers to higher level of care # of IFT forms completed Sample audits for completeness Interview 10 staff, physicians, EMS Are any unapproved or old forms being used?

  13. Report on the bi-annual Inventory Report - this is reviewed by the RPSRO Accountability Report on the Regional Quarterly Report Report at the RPSRO and RTAC
